To do or not to do? plasma exchange and timing of steroid administration in progressive multifocal leukoencephalopathy

2016

OBJECTIVE: To retrospectively analyze the effect of plasma exchange (PLEX; yes = PLEX+ , no = PLEX- ) and steroids administration timing (prophylactically [proST] or therapeutically [therST]) on the longitudinal clinical course of patients with natalizumab-related progressive multifocal leukoencephalopathy (PML) and full-blown immune reconstitution inflammatory syndrome (PML-IRIS). METHODS: Clinical and radiological data of 42 Italian patients with PML were analyzed. Patient's data are available until 12 months after PML diagnosis. PLEX and steroids treatment as time-dependent covariates were entered in: (1) a Cox model to investigate their impact on full-blown PML-IRIS latency; (2) an anal…

Light distortion of soft multifocal contact lenses with different pupil size and shape

2020

Purpose: To study the light distortion effects of soft multifocal contact lenses and the influence of different pupil size and shape. Methods: A total of 14 eyes of 7 healthy contact lens users (mean age 28.6 +/- 8.5 years) were recruited. The "Light Disturbance Analyser" device was employed for light characterization. The selected lenses were the monthly disposable "Biofinity multifocal" (CooperVision, CA, USA), both centre-distance and centre-near designs (addition power + 2.50 D). Two circular pupils of 3 mm and 5 mm of diameter, and an elliptical shape (3 horizontal and 5 vertical mm) were used. Metrics such as the light distortion index (LDI), best-fit circle radius and its irregularity…

Reversible posterior leukoencephalopathy secondary to indinavir-induced hypertensive crisis: A case report

2002

Reversible posterior leukoencephalopathy syndrome (RPLS) is an uncommon entity related to multiple and different pathologies, the most common being hypertensive crisis. It is believed to be secondary to the breakdown on the blood-brain barrier. At the beginning, it is undistinguishable from other leukoencephalopathies. However, the disappearance of brain lesions after removal of the potential cause, establish the differential diagnosis with other leukoencephalopathies. We present the case of an HIV-infected patient with a RPLS related to a hypertensive crisis short after the initiation of indinavir-containing highly active antiretroviral therapy. Once blood pressure was controlled and indin…

Refractive lens exchange with the Acri. Twin asymmetric diffractive bifocal intraocular lens system

2009

PURPOSE To evaluate efficacy, safety, and predictability after refractive lens exchange (RLE) in patients who had implantation of asymmetric Acri.Twin bifocal diffractive intraocular lenses. METHODS The study comprised 130 eyes of 65 consecutive patients after RLE who had bilateral implantation of the Acri.Twin system. Eyes were divided into myopic and hyperopic groups. Binocular uncorrected distance visual acuity, best-corrected distance visual acuity (BCVA), uncorrected distance near visual acuity, and best distance-corrected near visual acuity (BCNVA) were recorded preoperatively and 6 months after surgery. RESULTS Efficacy indexes were 0.99 for myopic and 0.93 for hyperopic patients at …

Comparison of short-term light disturbance, optical and visual performance outcomes between a myopia control contact lens and a single-vision contact…

2020

Purpose To compare the optical and visual performance of a dual-focus (DF) contact lens used for myopia control with a single-vision (SV) contact lens of the same material. Methods A randomised, double-masked crossover study. Twenty-eight healthy, myopic volunteers between 18 and 32 years of age (23.49 +/- 4.07 years) participated in this study. The sensory dominant eye for distance vision was determined. Refraction, best-corrected visual acuity (VA) and aberrations for 3 mm and 5 mm pupil diameters were quantified without a contact lens in situ. Subjects were fitted with two contact lens designs: DF and SV, both made of omafilcon A material. The Quality of Vision (QoV) questionnaire, over-…

Treatment of relapsing-remitting multiple sclerosis after 24 doses of natalizumab: evidence from an Italian spontaneous, prospective, and observation…

2014

Importance The evaluation of therapeutic choices is needed after 24 doses of natalizumab in patients with multiple sclerosis (MS). Objective To evaluate the effect of therapeutic choices on the mean annualized relapse rate and on magnetic resonance imaging MS activity after 24 doses of natalizumab in patients with relapsing-remitting MS. Design, Setting, and Participants The TY-STOP study, which recruited participants between October 22, 2010, and October 22, 2012, at 8 Italian MS centers (secondary care outpatient clinics) among 124 adult patients who demonstrated no clinical or magnetic resonance imaging MS activity after 24 doses of natalizumab. Interventions Natalizumab, no treatment, i…

Refractive lens exchange with an array multifocal intraocular lens.

2002

ABSTRACT PURPOSE: To prospectively evaluate safety, efficacy, predictability, stability, complications, and patient satisfaction after refractive lens exchange (clear lens extraction) followed by posterior chamber implantation of a multifocal intraocular lens (IOL). METHODS: Fifty eyes of 25 patients (mean age 51 years, range 44 to 62 years) with preoperative spherical equivalent refraction between -15.50 and +5.75 D and cylinder between 0 and 1.50 D underwent bilateral implantation of a zonal progressive multifocal IOL (Array, AMO). RESULTS: Eyes were divided into group A (n=24; myopia, average preoperative spherical equivalent refraction -7.11 ± 3.25 D (-1.75 to -15.50 D), and group B (n=…

Synchronous parenchymal renal tumors of different histology in the same kidney

2009

Background: Synchronous primary cortical tumors of different histology in the same kidney are rare. Preoperative awareness of this coexistence is important, since it can help in planning the surgical approach. Purpose: To describe the imaging findings observed in five patients with two parenchymal malignant tumors of different histology in the same kidney. Material and Methods: We reviewed the pathology reports of 381 patients operated for renal tumors from January 2000 to March 2007. The medical records of all patients with multiple, synchronous, primary lesions of different histology in the same kidney were reviewed with special attention to imaging findings and indications on the nature…

Differences in visual performance of AcrySof ReSTOR IOL in high and low myopic eyes.

2009

PURPOSE To determine the differences in refractive and visual results obtained in eyes with high and low myopia after multifocal intraocular lens (IOL) implantation. METHODS Monocular visual acuity for distance and near, with and without distance correction, and photopic and mesopic contrast sensitivity were measured at 6 months after surgery in 152 myopic eyes implanted with the AcrySof ReSTOR Natural (SN60D3) IOL. Two groups were created: high and low myopia (76 eyes in each). RESULTS Monocular best distance-corrected visual acuity was 0.02+/-0.06 logMAR for low myopes and 0.05+/-0.09 logMAR for high myopes. Monocular best distance-corrected near visual acuity was 0.04+/-0.05 and 0.07+/-0…

Tear film stability over a myopia control contact lens compared to a monofocal design

2021

Background: To compare the short-term effect of two contact lenses on pre-lens tear film stability and comfort: dual-focus contact lens (MiSight) and a monofocal contact lens (Proclear 1-day).Methods: This randomised, double-masked, crossover study was performed in twenty-eight healthy, myopic volunteers aged between 18 and 32 years (23.5 +/- 4.1 years). Only one randomly chosen eye was assessed. Distance vision and refraction were evaluated at baseline. Each contact lens type (monofocal and dual-focus) was randomly fitted, always in both eyes. A visual analogue scale between 0 and 10 was used to assess general comfort, physical comfort, and visual comfort. Tear Film Surface Quality (TFSQ) …
