Clinical evaluation of piezoelectric ear surgery

We evaluated the use of piezoelectric surgery (Piezosurgery; Mectron Medical Technology; Carasco, Genoa, Italy) as a means of avoiding some complications of osteotomy and osteoplasty in otologic surgery, particularly in classic canal-wall-up mastoidectomy. Piezoelectric surgery is a recently developed system for cutting bone with microvibrations created by the piezoelectric effect. This effect occurs when an electric current is passed through certain ceramics and crystals, causing them to oscillate at ultrasonic frequencies. Effects of botulinum toxin type A on vibration induced facilitation of motor evoked potentials in spasmodic torticollis.

It has not been clarified if botulinum toxin (BTX) injection leads to muscle spindle dysfunction in man. Neck Muscle Vibration and Spatial Orientation During Stepping in Place in Humans

Unilateral long-lasting vibration was applied to the sternomastoid muscle to assess the influence of asymmetric neck proprioceptive input on body orientation during stepping-in-place. Blindfolded subjects performed 3 sequences of 3 trials, each lasting 60 s: control, vibration applied during stepping (VDS), and vibration applied before stepping (VBS). VDS caused clear-cut whole body rotation toward the side opposite to vibration. The body rotated around a vertical axis placed at about arm's length from the body. Management of cerebrospinal fluid otorrhea.

Abstract Introduction Cerebrospinal fluid otorrhea results from an abnormal communication between the subarachnoid space and tympanomastoid compartment; most of them are of traumatic aetiology. They have clinical interest due to the potential risk of meningitis, directly related to the aetiology. Our aim was to show our experience in the management of this process. Management of chronic otitis by middle ear obliteration with blind sac closure of the external auditory canal.

Objective: Description of a technique of middle ear obliteration (MEO) with blind sac closure of the external auditory canal with discussion of the indications for its use in cases of recalcitrant chronic otitis and in far advanced disease. Patients: All patients underwent otologic examination and audiologic and radiologic assessments in a quaternary center. Results: Fifty-three cases of MEO were analyzed. For 9 patients, primary surgery was performed. One case of residual disease was identified. The minimum follow-up was 2 years. Ultrasonography for the diagnosis of Lyme disease in cases of acute facial paralysis.

Ultrasonography of the parotid gland was introduced in 1989 as part of the diagnostic protocol of every patient treated in our clinics with acute facial paralysis. Ten of 50 patients so tested were found to have unilateral nonpalpable enlarged lymph nodes in the caudal portion of the parotid gland around the stylomastoid foramen. All 10 patients were eventually diagnosed as having Lyme disease. It is suggested that in patients with acute facial paralysis, ultrasonography is an inexpensive tool to improve diagnosis of Lyme disease and may have a more useful purpose in monitoring therapy.

Vestibular-evoked myogenic potentials in "vestibular migraine" and Menière's disease: a sign of an electrophysiological link?

Characterizations of the signs and symptoms of "vestibular migraine" and of Meniere's disease seem to overlap, suggesting that both diseases might be due to a common peripheral vestibular dysfunction. Thus, the aim of the present study was to assess vestibular-evoked myogenic potentials (VEMPs) in both disorders to determine whether there might be an electrophysiological link between the two disorders. The authors describe two techniques for reconstruction of these areas after cancer excision with local flaps.
