Three-year (2014-2016) activity report of the Replantation Service for hand amputations in Poland

W pracy przedstawiono podsumowanie 32 miesięcy działalności Serwisu Replantacyjnego dla amputacji rąk w Polsce od stycznia 2014 roku do sierpnia 2016 roku. W analizowanym czasie zgłoszono łącznie 568 przypadków – amputacji, częściowych amputacji i innych rozległych urazów kończyn górnych, z czego zaakceptowano 354 przypadki (62%), a odrzucono 214 (38%). Wśród 354 wszystkich zaakceptowanych przypadków znalazło się 167 całkowitych amputacji (47%), 142 amputacji niecałkowitych (40%) i 45 (13%) innych ciężkich urazów, które zagrażały utratą kończyny. To evaluate visual and optical quality of the implantable collamer lens for different powers and sizes of incision surgery.Prospective study in humans.An adaptive optics visual simulator was used to measure 3 powers of implantable collamer lenses and simulate the implantable collamer lens wavefront aberration's pattern for small- and large-incision surgery. Visual acuity (VA) and contrast sensitivity were measured in 11 observers for 3- and 5-mm pupils. Repair of the mandibular nerve by means of autologous nerve grafting after resection of the lower jaw

Summary On the basis of two cases we demonstrate our method of autologous nerve grafting for substitution of the mandibular nerve after mandibular resection. The sural nerve served as a donor nerve, the graft was imbedded microsurgically at the juncture points by means of a perineurial interfascicular nerve suture. Oversized grafts of about 20 cm in length were chosen intentionally in order to insert them without tension between the stumps of the recipient nerve and outside the regeneration zone of the bone. In both cases complete resensibilization of the lower lip was reached after about six months.
Endoscopic Treatment of Mesencephalic Ependymal Cysts: Technical Case Report

OBJECTIVE: To evaluate the results of different neuroendoscopic surgical procedures for the treatment of mesencephalic ependymal cysts. METHODS: The clinical records of five patients treated for symptomatic mesencephalic ependymal cysts were retrospectively studied. Two patients had been operated on using an endoscope-assisted microsurgical technique and three patients using a pure endoscopic procedure. RESULTS: Adequate fenestration of the cysts was achieved in all of the patients. Two of the patients were symptom-free, and the other three had improved significantly after a mean follow-up duration of 14 months. There was no surgical morbidity. In this article, the authors present their own experience in vulvar reconstruction following vulvectomy using two different innervated island flaps according to the size and site of the defect. Island-flap mobilization is possible thanks to the rich blood supply of the perineal region. The methods described are a "V-Y amplified sliding flap from the pubis" and a "fasciocutaneous island flap" raised from one or both gluteal folds. The V-Y amplified sliding flap from the pubis is indicated when the defect is symmetric and located anteriorly. A preclinical cadaver study was performed to develop the technique of biportal neuroendoscopic dissection in the subarachnoid space of the basal cisterns and to test the feasibility, utility, and safety of this new technique. In 23 fresh post-mortem adult human cadavers and 2 formalin-fixed adult human head specimen a total of 33 biportal endomicrosurgical dissections into and within the basal cisterns were carried out. Peritumoral Edema in Meningiomas

Although generally benign tumors, meningiomas may be associated with extensive peritumoral brain edema as seen on computed tomographic scans. Fifty-two patients with intracranial meningiomas were studied, and the hypodense areas on computed tomographic scans were related to the intraoperative microsurgical findings and to the sizes of the tumors. We have identified three kinds of tumor-brain interfaces characterized by different difficulties in microsurgical dissection: smooth type, transitional type, and invasive type.
