Three-year (2017-2019) activity report of the Replantation Service for hand amputations in Poland

W pracy przedstawiono podsumowanie działalności Serwisu Replantacyjnego dla amputacji rąk w Polsce w latach 2017-2019. W analizowanym okresie, do Serwisu zgłoszono 551 przypadków amputacji i innych rozległych urazów kończyn górnych, z czego 330 zaakceptowano do operacji, a w 221 przypadkach, z różnych powodów odmówiono przyjęcia. Wśród 330 przyjętych było 165 (50%) amputacji całkowitych, 131 (40%) prawie całkowitych i 34 (10%) inne, ciężkie, zagrażające utratą kończyny urazy. W przeważającej liczbie ofiarami byli mężczyźni w młodym i średnim wieku. Objective: To present a new simulator designed at the Minimally Invasive Surgery Centre (Cáceres) which offers an integral, effective and viable training system that can be used for learning microsurgical techniques. Study design: A square methacrylate block was used. Animal jaws were fixed to the sides by means of butterfly screws. This block also has a drainage tube to facilitate the escape of fluids. A case of a suprasellar granular cell tumor, approximately 1.5 cm in diameter, in a 68-year-old woman is described. Diagnosis was established postoperatively by histopathological examination of the tumor tissue. The preoperative computed tomographic scan revealed a slightly hyperdense suprasellar mass with strong contrast enhancement. There was no evidence of calcification. The T1-weighted image on magnetic resonance imaging scan showed an isointense tumor with non-homogeneous enhancement after intravenous gadolinium diethylene-triamine-pentaacetic acid. Subarachnoid hemorrhage (SAH) occurring after an intracranial aneurysm rupture has an incidence of 10.5 per 100,000 person-years and accounts for about 5% of strokes. The resulting outcome depends on several factors including the severity of the initial hemorrhage, rebleeding, perioperative medical management, and the timing and technical success for aneurysm treatment. The overall mortality rates from 32%−67% with 10%–20% of patients with long-term dependence due to brain damage. Seventy-five patients with sessile adenomas or early carcinomas of the rectum or rectosigmoid were operated on with the new technique "transanal endoscopic microsurgery" Employing a newly developed complex endoscopic operating system, complete removal of sessile adenomas can be accomplished up to a distance of 25 cm from the anal verge, accurately and non-invasively.
