Impact of obstructive sleep apnea on metabolic dysfunction in severe obesity.

Obstructive Sleep Apnoea (OSA) seems to worsen metabolism. This effect has not evaluated in morbid obesity (MO). We hypothesized that the metabolic profile is more impaired in MO patients with OSA than in those without and investigated whether any specific metabolic dysfunction is related to OSA in MO. A prospective multi-centre cross-sectional study was conducted in consecutive subjects before bariatric surgery. OSA was defined as apnoea-hypopnoea index (AHI)≥15 by overnight polysomnography (PSG). Morbid obesity has become a global pandemic in recent years: 5% of the adult population of the USA are considered morbidly obese, i.e., have a BMI that exceeds 40. Every 90 seconds a US American dies of obesity and its consequences – making 1,000 people every day and 400,000 a year. This explains the dramatic rise of interest in bariatric procedures and with it in suitable anesthesiological methods.

We thank Aydin & cols. for their comments about our recently published paper “Red blood cell distribution width is not related with inflammatory parameters in morbidly obese patients” in Clinical Biochemistry Journal [1]. The authors show their concern about the clinical interpretation of our findings since we did not report folate and B12 vitamin levels. Moreover, they state that we should demonstrate the elimination of thrombocytopenic diseases by showing platelet count data. Aswe state in the paper, exclusion criteria for obese patients included “organic, malignant, hematological, infectious or inflammatory disease”. Gastric Banding for Treatment of Morbid Obesity: Preliminary Results.

Background: gastric banding (GB) has been used for treatment of morbid obesity. Methods: a banding device, introduced by Broadbent and consisting of a self-blocking nylon strip covered with a silicone tube, was used in 13 patients who have completed 1-year follow-up. This device was used for its mechanical properties, biocompatibility, ease of insertion and low cost. Results: at 1 year, mean excess weight loss was 51.6%, with all but one patient losing more than 25% of excess weight. Associated illnesses resolved.
