Cavernous sinus metastasis from oropharyngeal squamous cell carcinoma.

Metastasis to the cavernous sinus from head and neck cancer is uncommon and has been previously reported by a few authors. It is usually a late manifestation of the primary tumor and may be the first evidence of a widespread dissemination of the disease. Main clinical findings are those related with involvement of cranial nerves III to VI as they pass through the cavernous sinus. Although diagnosis may be difficult, the appearance of clinical and radiological findings of cavernous sinus involvement in a context of head and neck cancer must alert us about an intracranial metastatic infiltration. The lichen planus is a mucocutaneous disease with unknown etiology and auto-immune pathogenia. There have been three variants of lichen planus: the reticular, the plaque-like and the atrophic-erosive lesions. It?s a chronic disease with acute relapses that generally affects more frecuently to women from the fourties. The diagnostic is based on the clinic identification of the lesions joined with the histopathologic study (basal cells hidropic degeneration, linfoplasmocitic infiltration and absence of displasy signs). Oral lichen planus (OLP) has a prevalence of approximately 1%. The etiopathogenesis is poorly understood. The annual malignant transformation is less than 0.5%. There are no effective means to either predict or to prevent such event. Oral lesions may occur that to some extent look like lichen planus but lacking the characteristic features of OLP, or that are indistinguishable from OLP clinically but having a distinct cause, e.g. amalgam restoration associated. Such lesions are referred to as oral lichenoid lesions (OLLs). The management of OLP and the various OLLs may be different. Therefore, accurate diagnosis should be aimed at.

Abstract Lichen planus is an autoimmune, itching disorder that involves T lymphocytes, which may interest the skin, mucous membranes and exoskeletons. The presence of lichen manifestations both at the skin, mucous membranes and at the level of the skin attachments (hair and nails) is rarely mentioned in the literature.
