Cirugía de tumores retroperitoneales con afectación de vena cava: revisión de 18 casos

Resumen Introducción El tumor retroperitoneal con extensión a la vena cava inferior (VCI) más frecuente es el carcinoma de células renales. Tiene una supervivencia a 5 años de hasta un 32% cuando se realiza nefrectomía radical y trombectomía. El tratamiento quirúrgico es complejo y puede necesitar el soporte de circulación extracorpórea y parada circulatoria e hipotermia. Métodos Estudio descriptivo, retrospectivo, mediante análisis de datos de pacientes intervenidos de tumores con invasión de la VCI entre 1992 y 2015. Das akute idiopathische Skrotalodem (AISE) wird als eine seltene Ursache fur das Akute Skrotum angesehen. Nach Ausschluss der haufigeren Ursachen (Hoden- und Hydatidentorsion, Epididymitis, Hernie oder Hydrozele) erlauben die typischen klinischen Symptome und die charakteristischen Befunde bei der Dopplersonografie eine sichere Diagnosestellung, die in den meisten Fallen zur Vermeidung unnotiger Hodenfreilegungen beitragen kann.

Manganese effects on haematopoietic cells and circulating coelomocytes of Asterias rubens (Linnaeus)

Abstract Manganese (Mn) is a naturally abundant metal in marine sediments where it mainly occurs as MnO 2 . During hypoxic conditions it is converted into a bioavailable state, Mn 2+ , and can reach levels that previously have shown effects on immune competent cells of the crustacean, Nephrops norvegicus . Here we investigated if Mn also affects circulating coelomocytes and their renewal in the common sea star, Asterias rubens , when exposed to concentrations of Mn that can be found in nature. When the sea stars were exposed to Mn it accumulated in the coelomic fluid and the number of circulating coelomocytes, in contrast to what was recorded in Nephrops , increased significantly. These are designated as the “glomerular type=G type” (essentially identical with thrombotic microangiopathy), as “arterial type=A type” (essentially identical with primary malignant nephrosclerosis (PMN)) or as “mixed type=G/A type”. One year prior to presentation the patient suffered a transitory ischaemic brain attack concomitant with the onset of arterial hypertension, which was subsequently treated with calcium antagonists and beta blockers. Purpura-like skin lesions were noted which were reported to have occurred intermittently over a period of 5 years on both legs and occasionally on the arms. Proteinuria and haematuria were noted during periods with skin lesions.

Combined homozygous factor H and heterozygous C2 deficiency in an Italian family

Three of four children in a family have homozygous (less than 1% of normal) deficiency of factor H of the complement system and both parents, who are first cousins, are heterozygous for the same defect. The father and two of the H-deficient siblings also have a partial C2 deficiency. One of the children with combined deficiencies is affected by systemic lupus erythematosus with nephritis. No increased susceptibility to infections has been observed in the family. H deficiency is inherited in an autosomal codominant manner and is independently transmitted from C2 deficiency and HLA haplotypes.
