Transverse fracture-dislocation of the sacrum: a diagnostic pitfall and a surgical challenge.

Transverse fracture-dislocations of the sacrum are rare. Associated lesions of the lumbosacral spine as well as neurological injuries are common. Conventional radiographs of the pelvis often fail to clearly visualize the fracture. Delayed diagnosis increases the risk of progressive neurological disfunction. True lateral sacral views and CT-scans with 3-dimensional reconstructions are very helpful in establishing the full extent of the injury. These examinations should be considered in all patients with a history of high energy trauma and clinical signs indicating lumbosacral injury, such as severe low back pain and neurological disturbances of the lower extremities. The "critical zones" of entrapment of the nerves of the lower limb.

The author has studied, in a group of 40 dissections on cadavers of individuals of different ages, the main “critical zones” of entrapment of some terminal branches of the lumbo-sacral plexus, which include canals (fibrous, osteo-fibrous, fibro-muscular), intervals (intermuscular, fibro-muscular, musculo-ligamentous), rings (fibrous or fibro-muscular) and foramina. They provide the topographical anatomical basis for possible compressive phenomena of the nerves of the lower limb.

Mechanical activity of small and large intestine in normal and mdx mice: a comparative analysis.

The aim of this study was to compare the motor pattern (recorded as changes in intraluminal pressure) of isolated duodenum and proximal colon between dystrophic mdx and normal mice. When duodenal recordings from control preparations were compared with mdx mice there was no significant difference in the spontaneous motor pattern, responses to electrical nerve stimulation or sensitivity to pharmacological agents. Colonic segments from mdx mice showed a more complex motor pattern, consisting of contractions with amplitude and frequency similar to those of controls and by additional contractions with lower amplitude and higher frequency. Duodenal contractile activity in dystrophic (mdx) mice: reduction of nitric oxide influence.

The present study was undertaken to analyse duodenal contractility in adult dystrophic (mdx) mice. The spontaneous changes of the isometric tension and the responses of longitudinal duodenal muscle to nonadrenergic, noncholinergic (NANC) nerve stimulation and to exogenous drugs were compared between normal and mdx mice. Duodenal segments from mdx mice displayed spontaneous contractions with higher frequency than normals. N omega-nitro-L-arginine methyl ester (L-NAME) increased the frequency of contractions in normals without affecting that in mdx mice. Anatomy of the Sacral Roots and the Pelvic Splanchnic Nerves in Women Using the LANN Technique

AIMS: To report on our anatomic and electrophysiologic findings about the sacral nerve roots and the pelvic splanchnic nerves during laparoscopic pelvic surgery. METHODS: The pelvic splanchnic nerves and the sacral nerve roots were dissected in 336 consecutive patients undergoing laparoscopy for pelvic pain syndrome or gynecologic diseases. Intraoperative assessment of the functionality of the exposed nerves was performed using the LAparoscopic Neuro-Navigation (LANN) technique. RESULTS: Dissection of the sacral roots and the splanchnic pelvic nerves lateral to the sacral hypogastric fascia was feasible without any complications in all patients in this series. Abstract Sciatic nerve damage is a well-known complication that occurs in 1.5% of patients after primary total hip arthroplasty and in 8% after revision total hip arthroplasty. Yet when considering re-revision arthroplasty and acetabular cage implantation, incidence and management remain unclear. Spätsynovektomie des Ellenbogens und Resektion des Radiusköpfchens bei chronischer Polyarthritis

Between 1974 and 1984 16 patients with rheumatoid arthritis, who had elbow synovectomy and excision of the radial head, were revised with an average follow-up of 5 years after operation. Resection of the radial head was performed on 16 elbows, in 3 elbows the ulnar nerve was transposed. The indication for the operation were pain, swelling, warmth and radiological destruction. The parameters used, to assess efficacy of the operation were range of motion, local finding and patient satisfaction. Synovectomy results were good especially in respect of relief of pain and range of motion. The good results justify elbow synovectomy even in the late stages of disease.
