Análisis de la mortalidad evitable en los hospitales de la Comunidad Valenciana

Using a list of avoidable mortality (AM) causes as indicators of medical care (IMC) according to the Holland classification, a study on AM was conducted in the Valencia Community hospitals during 1994 and 1995. A total of 617 patients out of 106,540 discharges with IMC criteria died (mortality rate: 0.58%). The most common causes of AM included hypertension and cerebrovascular disease, with 46% of avoidable deaths; maternalperinatal disease, with 36%, and tuberculosis, 7%. Pregnancy in advanced maternal age: maternal and fetal outcome.

The objective of this research is to evaluate maternal and fetal outcome of pregnant women over 40 years. The outcome has been evaluated through the following data: age, nationality, number of deliveries, complications of pregnancy, type of delivery, newborn's weight and APGAR index. 201 pregnant women have been recruited. The GDM has been observed in 11 women (5.5%). The antepartum haemorrhage has been observed in 7 women (3.5%). The cesarean section rate was 76% in the Group of primiparae woman and 64% in the Group of multiparae. The incidence of Children LBW (low Birth weight) was 17% in the primiparae and 10% in the multiparare.

We report on our experience with 7 pregnancies in 6 women who previously underwent reconstruction of the urinary tract with a continent urinary diversion using an ileo caecal segment (Mainz-Pouch I), ureterosigmoidostomy or bladder augmentation. Urinary tract infection and upper tract dilatation were the only complications during pregnancy which required bilateral nephrostomies in one case. All other sequelae were handled conservatively. In preeclampsia, increased water-sodium retention results from a variety of pathophysiological changes in the endocrinological system. There are also organ-specific changes, such as renal clearance reduction up to 30% in comparison to normal pregnancy. For many years, physiologists expected to find additional cardial mechanisms that influence homeostasis. In 1981, de Bold [3] identified ANP a new humoral factor that leads to increased water and sodium excretion. The influence of ANP in preeclampsia has been the subject of controversial discussion.

Die Perforation bei Morbus Crohn als Komplikation in der Schwangerschaft

The manifestation of Crohn's disease with perforation in pregnancy has been described in one case only. In our case, a 25-year-old woman in the 28th week of pregnancy was admitted to our hospital with signs of an acute abdomen. Due to deterioration of the maternal and foetal situation, the child was delivered by a Caesarean section. Subsequently, exploration of the abdomen showed an ileum perforation with diffuse fibrinous peritonitis. In this case, the definitive diagnosis could not be made prior to laparotomy.
