ABH-antigenicity of transitional cell carcinoma of the urinary bladder in patients subjected to topical chemoprophylaxis

Topical chemoprophylaxis with adriamycin was administered to 49 patients with UICC-stages pTA and PT1 (pathological stage 0 and A) of transitional cell carcinoma of the urinary bladder. Tumor specimens obtained prior to initiation of the instillation protocol were evaluated with regard to ABH-antigenicity. ABH-antigens were present in tumors of 19 patients and none of these developed muscle-infiltrating recurrences during or after intravesical chemoprophylaxis. The primary lesions of 30 patients were deleted of antigens; only 5 (16%) developed a progressive recurrence. Chronic renal failure: oxidative stress, endothelial dysfunction and wine.

Atherosclerosis development is accelerated in chronic renal failure (CRF) and is the major cause of death in this clinical condition. An increased oxidative stress and an endothelial dysfunction, with their complex interrelationships, are relevant aspects of atherogenesis in CRF patients and might be targets for treatment. Many studies have underlined the cardiovascular protection provided by a moderate wine consumption. This beneficial effect is due to both alcohol and nonalcoholic components of wine including several phenolic molecules such as quercetin and resveratrol. Experience with palliative percutaneous nephrostomy in bladder cancer patients

Minimal percutaneous nephrostomy as a palliative urinary diversion was performed in 50 patients with advanced bladder cancer disease. The average survival time was 6.6 months, with 38% of the patients alive after 6 months. Although 88% of the patients died within one year, 2 (5%) are still alive after more than 2 years. Each case of palliative diversion is an individual one, with many different factors influencing the decision. Prolonging life cannot be an indication for diversion. Percutaneous nephrostomy in combination with other percutaneous techniques provides the urologist with excellent therapeutic tools to avoid surgery in such emergency situations as uremia or local tumor symptoms. …

Wilms' tumor ? treatment and prognosis of 373 children

Between June 1980 and June 1988, 373 patients with Wilms' tumors were diagnosed and treated (34.5%, stage I; 34.5%, stage II; 14.8%, stage III; 10.7%, stage IV; and 5.4%, stage V). In all, 11% of the patients were diagnosed as a result of routine checkup examinations. The tumor volume was 400 ml in 52.7%. Overall, 78.7% of the patients had a standard-type Wilms' tumor, 7% had a low-grade malignancy lesion and 14.3% had a high-grade-malignancy tumor. Aside from radical tumor nephrectomy, treatment encompassed chemotherapy in all cases and radiotherapy in half of the patients according to the study protocol. A reinforcement learning approach for individualizing erythropoietin dosages in hemodialysis patients

This paper presents a reinforcement learning (RL) approach for anemia management in patients undergoing chronic renal failure. Erythropoietin (EPO) is the treatment of choice for this kind of anemia but it is an expensive drug and with some dangerous side-effects that should be considered especially for patients who do not respond to the treatment. Therefore, an individualized treatment appears to be necessary. RL is a suitable approach to tackle this problem. Low-frequency extracorporeal shock wave lithotripsy improves renal pelvic stone disintegration in a pig model.

OBJECTIVE To compare disintegration rates for renal stones treated by 60 vs 120 shock waves (SW)/min at the same energy settings, using standardized validated artificial stones in a pig model. MATERIALS AND METHODS Gypsum artificial stones (13 × 6 mm) were inserted into the renal pelvis on either side of 12 anaesthetized pigs by open surgery. Extracorporeal SW lithotripsy (ESWL) was applied using a new electromagnetic lithotripter (Lithoskop®, Siemens AG Healthcare, Munich, Germany) at 60 and 120 SW/min; 3000 SW were applied to each kidney with the same energy settings. Parenchyma-sparing surgery for renal tumors

Between 1969 und 1989 conservative surgery for kidney tumors was performed in 123 patients. In 49 patients there was an imperative indication for a parenchyma-sparing operation because nephrectomy would have made dialysis obligatory. Thirty-five of these 49 patients show no signs of tumor progression after a mean follow-up of 4.5 years. Known metastases were present in 3 of 6 patients who died as a result of their tumors. In two patients there was a recurrence after 1 and 5 years, respectively, requiring a second organ-preserving operation.
