Use of corticosteroids in Coronavirus disease 2019 pneumonia: A systematic review of the literature

2020

The aim was to investigate the effectiveness of glucocorticoid therapy in patients with COVID-19. A systematic search of the literature across nine databases was conducted from inception until 15th March 2020, following the PRISMA guidelines. Patients with a validated diagnosis of COVID-19 and using corticosteroids were included, considering all health outcomes. Four studies with 542 Chinese participants were included. Two studies reported negative findings regarding the use of corticosteroids in patients with COVID-19, i.e., corticosteroids had a detrimental impact on clinical outcomes. One study reported no significant association between the use of corticosteroids and clinical outcomes. …

Perioperative management of patients with suspected or severe infection with SARS-CoV-2 coronavirus programmed for the implementation of electronic d…

2020

At the end of 2019, a series of patients affected by lung infection of initially unknown aetiology with clinical presentations very similar to viral pneumonia were registered in China. Sequencing analysis of samples from the lower respiratory tract identified a new type of virus from the family Coronaviridae called SARS-CoV-2 as the causative agent of the outbreak; the agent responsible for the disease that was renamed COVID-19.1 Since then, millions of cases have been identified around the world. The World Health Organization (WHO) initially declared the infection to be a Public Health Emergency of International Concern, and later classed it as a pandemic.2, 3 The vast majority of coronavi…

Patógenos atípicos en pacientes hospitalizados con neumonía adquirida en la comunidad: una perspectiva mundial

2018

Abstract Background Empirical antibiotic coverage for atypical pathogens in community-acquired pneumonia (CAP) has long been debated, mainly because of a lack of epidemiological data. We aimed to assess both testing for atypical pathogens and their prevalence in hospitalized patients with CAP worldwide, especially in relation with disease severity. Methods A secondary analysis of the GLIMP database, an international, multicentre, point-prevalence study of adult patients admitted for CAP in 222 hospitals across 6 continents in 2015, was performed. The study evaluated frequency of testing for atypical pathogens, including L. pneumophila, M. pneumoniae, C. pneumoniae, and their prevalence. Ris…

Circulating microRNAs can identify endotypes of community-acquired pneumonia

2019

Background: The identification of the host processes dysregulated in community-acquired pneumonia (CAP) patients, especially in those who develop severe complications could be crucial for future management of this disease. We aim to study microRNAs profiles to define different CAP endotypes regarding complications. Methods: An observational prospective study of consecutive hospitalized CAP cases was performed. Circulating microRNAs were analyzed using qRT-PCR. We study correlations and predictive value of miRNAs regarding severe sepsis and acute hypoxemic respiratory failure (AHRF: PaO2/FiO2 Results: We analyzed clinical data and blood samples from 169 CAP patients. The mean age was 66.9 ye…

A systematic review on the efficacy and safety of chloroquine for the treatment of COVID-19

2020

Purpose COVID-19 (coronavirus disease 2019) is a public health emergency of international concern. As of this time, there is no known effective pharmaceutical treatment, although it is much needed for patient contracting the severe form of the disease. The aim of this systematic review was to summarize the evidence regarding chloroquine for the treatment of COVID-19. Methods PubMed, EMBASE, and three trial Registries were searched for studies on the use of chloroquine in patients with COVID-19. Results We included six articles (one narrative letter, one in-vitro study, one editorial, expert consensus paper, two national guideline documents) and 23 ongoing clinical trials in China. Chloroqui…

SARS-CoV-2 and the Risk Assessment Document in Italian Work: Specific or Generic Risk Even If Aggravated?

2021

In December 2019, several cases of atypical pneumonia were detected in Wuhan city, Hubei province, inland China. The initial outbreak was of considerable size first in China subsequently spread to the rest of the world. Immediately after the epidemic (which according to the World Health Organization had risen to pandemic status), the problem of whether or not to update the occupational risk assessment arose, also considering how the biological risk from SARS CoV-2 should be understood: specific or generic. To this end, we conducted a literature review to identify national health legislation and policies, examining how Italy has addressed the COVID-19 emergency in occupational health plannin…
