Research into complex regional pain syndrome (CRPS) has made significant progress. First, there was the implementation of the official IASP "Budapest" diagnostic criteria. It would be desirable to also define exclusion and outcome criteria that should be reported in studies. The next step was to recognize the complex pathophysiology. The novel adamantane derivative APICA (N-(adamantan-1-yl)-1-pentyl-1H-indole-3-carboxamide) was recently identified as a cannabinomimetic indole of abuse. Oligodendrocyte ablation triggers central pain independently of innate or adaptive immune responses in mice.

Mechanisms underlying central neuropathic pain are poorly understood. Although glial dysfunction has been functionally linked with neuropathic pain, very little is known about modulation of pain by oligodendrocytes. Here we report that genetic ablation of oligodendrocytes rapidly triggers a pattern of sensory changes that closely resemble central neuropathic pain, which are manifest before overt demyelination. Biological Causes of Anterior Knee Pain

We review the pathophysiology of anterior knee pain in the young patient. Emphasis is placed on newer findings. We have developed what we call the “Neural Model” as an explanation for the genesis of anterior knee pain. We have demonstrated a neuroanatomical basis for PFPS in the young patient and the clinical observation that the lateral retinaculum may have a key role in the origin of this pain.
