Activation of nuclear factor erythroid 2-related (Nrf2) system as a novel therapeutic approach in COPD
2019
Background: Chronic Obstructive Pulmonary Disease (COPD) is an inflammatory lung disease caused by chronic exposure to cigarette smoke. Oxidative stress is one of the most important mechanisms involved in the physiopathology of COPD. Nuclear Factor Erythroid 2-related (Nrf2) is critical in protection against oxidative stress by inducing expression of antioxidant genes and a decrease in its expression has been observed in COPD patients. Objective: The aim of this study was to characterise the effects of Bardoxolone, Omavexolone and Obacunone as antioxidant drugs in COPD. Methods: Peripheral blood neutrophils from COPD and healthy volunteers and Primary Human Bronchial Epithelial cells were i…
LSC Abstract – Functional metagenomics of respiratoy microbiome in exacerbated COPD
2015
Background: The course of COPD is frequently aggravated by exacerbations. New molecular techniques have suggested that many bacterial groups, not only the common COPD pathogens, could trigger the exacerbations. Objective: to analyze the microbial community and the gene content of samples obtained during stability and exacerbation of COPD patients. Method: 16S rRNA was pyrosequenced to obtain the taxonomic information. The metabolic information was obtained with the Metagenomics RAST server (MG-RAST) with KEGG database. Results: 8 severe COPD patients were included. At genus level, 68 different OTUs were found. No significant differences in the relative abundance of any of the detected gener…
Factors associated with Pneumocystis colonization and circulating genotypes in chronic obstructive pulmonary disease patients with acute exacerbation…
2021
Abstract Pneumocystis jirovecii colonization is frequent during chronic obstructive pulmonary disease (COPD) and patients constitute potential contributors to its interhuman circulation. However, the existence of an environmental reservoir cannot be excluded. We assessed the prevalence and factors associated with Pneumocystis colonization during COPD, and studied circulation between patients and their domestic environment. Pneumocystis molecular detection and mtLSU genotyping were performed in oro-pharyngeal washes (OPW) sampled in 58 patients with COPD acute exacerbation, and in indoor dust, sampled in patients’ homes using electrostatic dust collectors (EDCs). Lung and systemic inflammati…
TLR4 and NOD1 increase in stable COPD of increasing severity. Relationship with tissutal bacterial load
2016
Background: The immune host response related to bacterial and viral infections in the airways and lung of COPD patients is unclear. Objectives: To investigate the expression of anti-bacterial and anti-viral antigens in bronchial biopsies and lung parenchyma of stable COPD patients in relation to bacterial load. Methods: Immunohistochemical (IHC) and qRT-PCR-expression of TLR2-3-4-7-8-9, NOD1, NOD2, MYD88, TRIF, TIRAP, pIRAK1, IRAK4, IRF3, pIRF3, IRF7, pIRF7, RIG1, MDA5, LGP2, MAVS, STING, DAI, IFNα and IFNβ was measured in bronchial mucosa in patients with mild/moderate (n=16), severe/very severe (n=18) stable COPD, control smokers (n=12) and control non-smokers (n=12). Selected relevant an…
Design and evaluation of a 32-channel phased-array coil for lung imaging with hyperpolarized 3-helium
2010
Imaging with hyperpolarized 3-helium is becoming an increasingly important technique for MRI diagnostics of the lung but is hampered by long breath holds (>20 sec), which are not always applicable in patients with severe lung disease like chronic obstructive pulmonary disease (COPD) or α-1-anti-trypsin deficiency. Additionally, oxygen-induced depolarization decay during the long breath holds complicates interpretation of functional data such as apparent diffusion coefficients. To address these issues, we describe and validate a 1.5-T, 32-channel array coil for accelerated 3He lung imaging and demonstrate its ability to speed up imaging 3He. A signal-to-noise ratio increase of up to a factor…
Cigarette smoke alters the EZH2/DAB2IP expression in bronchial epithelial cells. A risk factor for lung cancer in COPD patients
2016
Cigarette smoke is an environmental risk factor for COPD and Lung cancer, leading causes of morbidity and mortality worldwide. In cancer, enhancer of zeste homolog 2 (EZH2) silenced disabled homolog 2 interacting protein gene (DAB2IP) (tumor-suppressor gene) bytri-methylation of lysine 27 on histone H3 (H3K27me3). We studied EZH2 and DAB2IP expression in airway epithelial cells from COPD patients and their potential involvement in the progression of COPD toward lung cancer. We assessed EZH2 and DAB2IPimmunoreactivity in bronchial epithelial cells from surgical specimens of COPD patients and healthy control subjects (HC) by immunohistochemistry. Bronchial epithelial cell line (16HBE), primar…
Extracorporeal shock waves increase markers of cellular proliferation in primary bronchial fibroblasts of COPD patients
2019
COPD is due to a remodeling of small airways and destruction of the lung parenchyma with loss of the alveolar attachment as a result of pulmonary emphysema. Treatment of connective tissue fibroblasts with extracorporeal shock waves (ESW) increases their cellular proliferation and differentiation. To date no studies are available on ESW treatment of human primary bronchial fibroblasts from COPD and control subjects. Primary bronchial fibroblasts were obtained from bronchial biopsies from three patients with mild/moderate COPD and 3 control smokers with normal lung function. After stabilization for 24h, 1 ml of cell suspension (106 cells) from COPD and controls was treated in 2 ml tubes with …
A Study of the Effect of Proinflammatory Cytokines on the Epithelial Cells of Smokers, With or Without COPD
2011
Introduction: Cigarette smoke is the main cause of inflammation in COPD. The mechanisms that differentiate smokers who develop COPD are diverse. In this study, we analyzed the presence of cytokines in the respiratory secretions of smokers with or without COPD and the secretory properties of the differentiated bronchial epithelium obtained from the individuals themselves after exposure to tobacco smoke. Material and methods: Twenty-seven smokers were studied, 12 of whom had COPD that had not been previously treated with steroids. In 11, samples were obtained by means of induced sputum, and the remaining samples were collected from bronchial aspiration after bronchoscopy. Concentrations of IL…
2 years in the life of COPD patients: Evolution of GOLD 2011 classification in the 'real-life' DACCORD study
2016
Introduction Limited data are available on the progression of COPD categorised according to GOLD 2011. We analysed data from the ongoing, non-interventional, observational DACCORD study and compared baseline GOLD 2011 categorisation with that of 1 and 2-years after inclusion. Methods Patients with a diagnosis of COPD were recruited into DACCORD following either a change or initiation of maintenance medication. Baseline exacerbations data (collected over 6 months) were annualised for GOLD categorisation at baseline; COPD symptoms were evaluated using the COPD Assessment Test (CAT) at baseline as well as the 1 year and 2 year visit. Data on exacerbations were collected every 3 months up to 2 …
Automatic bilevel ventilation in sleep-disordered breathing: A real-life experience in southern Italy
2016
Automatic bilevel ventilation (AutoBI) has been recently introduced to treat obstructive sleep-disordered breathing (SDB), but clinical experience is still limited. We revised the charts of patients treated for obstructive SDB in our sleep clinic in 2015 (n=214) to assess the frequency of trials of AutoBI ventilators (ResMed Auto25 or Respironics BiFlex), the clinical characteristics of these patients, the reason to shift from CPAP to AutoBI, and the outcome of AutoBI titration. A trial of domiciliary AutoBI was made in 44 patients (20.5%) intolerant to CPAP due to high CPAP levels (n=11) or who showed incomplete resolution of SDB on CPAP (n=33). The table reports the clinical data accordin…
