Successful laparoscopic management of congenital diaphragmatic relaxation: A case report

Highlights • We describe a case report of congenital Diaphragmatic relaxation treated with totally laparoscopic plication of the left hemidiaphragm with nonresorbable sutures. • In our case report the diaphragmatic relaxation seemed to be congenital and the laparoscopic approach resulted the most appropriate technique to obtain the repositioning of abdominal organs and diaphragmatic placation. • A brief review of literature was performed to identify outcomes and potentially surgical advantages of laparoscopic approach for diaphragmatic relaxation. Laparoscopic approach can only be practiced in selected cases.

The management of antithrombotic drugs (antiplatelet and anticoagulant agents) is a common challenging issue in patients undergoing thoracic surgery. The procedure can require the temporary interruption of these drugs because of their associated haemorrhagic risk, therefore assuming a thrombotic risk also. The assessment of the balance between both risks gives us their optimal management.

RESUMEN En esta Tesis se propone un modelo simplificado de balance de energía de dos fuentes (STSEB) para el cálculo de los flujos energéticos en superficie, cuya principal característica es que los flujos de cada una de las fuentes se consideran totalmente desacoplados. Purpose To investigate the safety and efficacy of emergency treatment of acute aortic diseases with endovascular stent grafts. Methods In 11 patients (median age 55 years, range 18 - 85) with acute complications of descending aortic diseases endovascular emergency treatment was performed: traumatic aortic rupture (n = 4), penetrating ulcer with aortobronchial fistula or hematothorax (n = 4), acute type B dissection (n = 2, one with penetration, one with subacute mesenteric ischemia), and symptomatic aneurysm of the thoracic aorta (n = 1) with pain and diameter progression. 15 stent grafts were implanted (Talent n = 11, Vanguard/Stentor n = 4). Introduction: Tufted angioma (TA) is a rare benign vascular tumour that mostly appears during infancy or early childhood, although there are cases reported in adults. Clinical presentation and evolution of TA can vary. Histologically, it takes on a classic appearance of vascular tufts (“cannon ball” like appearance). Patients and methods: A retrospective observational study was conducted that included all patients diagnosed with TA at our centre in the last 20 years. Results: A series of 9 cases of tufted angioma in childhood are presented, 77.7% of which were congenital. This represents a frequency higher than previously described. Cystic Medial Necrosis of the Internal Thoracic Artery:Case Report

In an internal thoracic artery intended as a graft in coronary angioplasty, multiple intramural haematomas impeded blood flow and consequently precluded use of the artery as a free graft. The cause was found to be cystic medical necrosis of the internal thoracic artery. A good result was obtained with three single aorto-coronary venous bypass grafts.
