Abstract Background The long-term effectiveness of antipsychotic maintenance treatment after first-episode psychosis (FEP) is contested. In this real-world observational study, we examined how cumulative exposure to antipsychotics within the first 5 years from FEP was associated with the 19-year outcome. Epiphora Secondary to Blockage of the Lacrimal System: The Role of Endonasal Dacryocystorhinostomy

Stenosis of the lacrimal system may occur secondary to tumor, infection, trauma, and congenital etiologies. Attempts to remedy this problem often include irrigation and probing. However, if these fail, external or internal dacryocystorhinostomy (DCR) with or without stenting may be performed. The purpose of this article is to detail our experience with a microscopic endonasal approach to DCR utilized in 23 patients. This is a modification of an older accepted procedure which utilizes the modern techniques of endonasal sinus surgery. Primary anastomosis is becoming increasingly favoured because of the shorter hospital stay even in emergency operations on the colon and rectum. This appears entirely justified when an objective of cost-effective medicine has been set. At our hospital between September 1985 and February 1999, 365 patients were operated on because of diverticular disease. The data from 346 of these patients were evaluated. 202 of these cases were elective; 144 were carried out on an emergency or urgent basis. The recurrence rate after 438 curative operations for rectal carcinoma was 44.1% (193 cases). It was independent of age and sex, but correlated closely with tumour stage. Tumours in an early stage recurred rarely and late. The success of a second operation was influenced by the type of local-regional recurrence. Migration of a Fractured Acetabular Socket Wire Into the Femoral Vein: Indication for Urgent Removal by Venous Thrombectomy: A Case Report

The migration of a foreign body into a great vein is a rare occurrence that requires urgent surgical intervention. In our patient one fragment of a circular acetabular socket wire marker had entered the femoral vein. Exact topographic location of the wire was achieved by computerized tomography. Urgent surgery revealed a floating wire within the venous lumen with associated thrombosis.

Background: Pathogenesis of Granulomatosis with polyangiitis (GPA) is still unknown. However, it has been observed a skewing of circulating CD4+ T cells toward the Th17 and Th2 phenotype. The pro-inflammatory cytokine interleukin 25 (IL-25) is a member of IL-17 cytokine family associated to the Th2 immune phenotype. Through the receptor IL17RB, IL-25 further sustains the Th2-type immune response and elicits the expansion of the type 2 innate lymphoid cells (ILC2) and M2 macrophages. Breast cancer is the most commonly diagnosed malignancy in women in Poland. Rare, unusual forms of breast cancer remain a diagnostic problem. The incidence of ectopic breast glandular tissue in the general population varies between 0.4 and 6%. The same abnormalities and diseases as in the anatomical mammary gland may develop in this tissue. Breast cancer may develop as well. We present the case of a 36-year-old woman, who went to the doctor because of a nodule in the right armpit presented for 2 years. AbstractAbout ten years ago it was hoped that venous angioplasty for abnormal veins, primarily the internal jugular veins, will be a much awaited treatment for multiple sclerosis. Yet, a majority of randomized clinical trials on endovascular treatment for chronic cerebrospinal venous insufficiency did not reveal clinical efficacy of these procedures in multiple sclerosis patients.
