Skin-reducing Mastectomy with Primary Implant Reconstruction

Background: We present a series of skin-sparing mastectomies (SSMs) with skin reduction and immediate breast reconstruction to treat large and ptotic breasts. The technique combines oncological mastectomy with immediate subpectoral implant placement as a single-step procedure. Methods: Data was collected from a prospective database from February 2009 to April 2011. A total of 24 patients with macromastia or pronounced ptosis fulfilled the criteria for skin-saving mastectomy. Osteoporosis is “a systemic disease characterized by a reduction in bone mass and a microarchitectonic alteration of the skeleton, with a consequent increase in skeleton fragility and susceptibility to fractures”, as defined by the last Consensus Development Conference1. Objective: Patients with chronic, untreatable diseases have emotional needs that are often unaddressed by conventional care services within the healthcare systems. The situation of patients with idiopathic pulmonary fibrosis (IPF) is paradigmatic as there are few, if any, studies focusing on psychological interventions for these patients. This chapter describes a study that identifies and quantifies emotional distress, quality of life, spirituality, and key dimensions of health-related well-being after brief psychotherapy treatments at an outpatient clinic.

This is a report on a neonatal primary hyperparathyroidism (NPHPT) treated by total parathyroidectomy and autotransplantation of parathyroid tissue into the forearm musculature. Histologically, the glands were composed of a "chief cell hyperplasia" of the parathyroid tissue. Clinical manifestation in neonates and differential diagnosis of HPT are discussed. Indication for operation and surgical procedure are described.

Pressure-Induced Growth (PIG) of Atretic Esophagus: A Contigent Management for High-Risk Esophageal Atresia

The management of uncomplicated esophageal atresia is a straightforward procedure consisting of thoracotomy with anastomosis of the atretic segments and ligature of any tracheoesophageal fistula present. However, besides the rareness of isolated esophageal atresia, our observations in the past few years show increasing incidence of the anomaly in association with premature births and other malformations which are usually incompatible with life. Aspiration pneumonia resulting from reflux of gastric content through a lower tracheoesophageal fistula, rather than the overflow of saliva from an upper atretic pouch, also threatens the life of an otherwise healthy neonate with esophageal atresia.

Safety and efficacy of endovascular treatment for carotid artery stenoses using proximal protection systems : 30-day follow-up

Although surgical endarterectomy remains the treatment of choice for carotid artery stenosis, carotid artery stenting (CAS) with use of proximal protection systems (PPS) plays an important role as alternative treatment modality, especially in high risk patients. This study was aimed at the assessment of safety of CAS with use of the PPS and also at identification of risk factors associated with this procedure. Material and methods.
