Aims: chronic groin pain caused by an occult hernia can be a difficult clinical diagnosis and may require additional investiga- tive modalities. The aim of this study was to demonstrate the accuracy of ultrasound in diagnosing occult hernia in patients with recurrent inguinal pain and a negative clinical examination. Patients and methods: over a period of three years, a total of 76 patients were referred for ultrasound examination with clini- cally suspected occult inguinal hernia. Patients with positive US scans were offered surgery, while those with negative US findings were offered further imaging or other diagnostic tests. The data are produced by the ‘Global ozone monitoring of occultation of stars’ instrument on board the Envisat satellite that was launched in March 2002. The instrument measures the attenuation of light spectra at various horizontal paths from about 100 km down to 10–20 km. Ossification of the Achilles tendon is the result of a traumatic, hypoxic injury of the tendon. The usually asymptomatic ossification has a clinical importance only in case of a new trauma. We report about a patient with an isolated fracture of the ossification without an accompanying rupture of the Achilles tendon. Because of the radiologically "occult" fracture the diagnosis could only be verified by MRI. With a conservative treatment without resection of the ossification we could reach painless recovery.
Why Do We Have to Use Chemotherapy?

The use of chemotherapy (CT) in localized rectal cancer (LARC) has two aims: first, to improve the local effect of radiotherapy by giving concomitant chemoradiation and second, to decrease systemic relapses by early treatment for occult micrometastatic disease or to shrink bulky local tumours. Neoadjuvant treatment is reserved for locally advanced disease, as defined by pelvic magnetic resonance imaging, a very useful and accurate tool to identify high-risk features for local or systemic relapse [1]. We are going to address why we have to use CT in LARC.
