Cost-effectiveness of screening for colorectal cancer in France using a guaiac test versus an immunochemical test

2010

Objectives: The aim of this study was to compare the cost and the effectiveness of two biennial fecal occult blood screening tests for colorectal cancer: a guaiac nonrehydrated test (G-FOBT) and an immunochemical test (I-FOBT) with the absence of screening. Methods: A Markov model was developed to compare these strategies in a general population of subjects aged 50 to 74 over a 20-year period. Results: Compared with the absence of screening, G-FOBT and I-FOBT were associated with a decrease in colorectal cancer mortality of 17.4 percent and 25.2 percent, respectively. With regard to cost-effectiveness, expressed as cost per life-year gained, I-FOBT was the most effective and most costly alter…

The impact of occult renal failure on the cardiovascular risk stratification in an elderly population: the PREV-ICTUS study.

2008

To analyze the impact of occult renal failure (ORF) in the individual risk stratification and on the blood pressures (BP) and low-density lipoprotein (LDL) goals in an aged population, according to the ESH/ESC Hypertension Guidelines.A cross-sectional, population-based study on individuals aged 60 years or more carried out in Primary Care Centers of Spain. Kidney function was estimated from calculated creatinine clearance (eGFR), Cockroft and Gault formula. Ten-year cardiovascular risk was estimated through the ESH/ESC table including or not including the eGFR. Estimates of the modification in BP and LDL-cholesterol (cLDL) goals were calculated. In 6419 subjects, 4242 subjects (66%) had norm…

Intestinal melanoma: A broad spectrum of clinical presentation

2011

Abstract INTRODUCTION Small intestine melanomas are rare and the most of them are metastases from primary cutaneous neoplasms. PRESENTATION OF CASE Below, we report two cases of small intestine metastatic melanoma with very different clinical presentation. DISCUSSION Still now, primary versus metastatic origin is often unclear. Small bowel melanoma is often asymptomatic. However, clinical picture can be various; it may occurs with non specific symptoms and signs of gastro-intetstinal involvement, like chronic abdominal pain, occult or gross bleeding and weight loss, or with an emergency picture due to intestinal intussusception, obstruction or, rarely, perforation. CONCLUSION Small bowel me…

Screening for colorectal cancer with immunochemical faecal occult blood tests.

2012

Population-based studies have shown that guaiac faecal occult blood testing followed by colonoscopy in case of positivity can reduce colorectal cancer mortality. However these tests have been criticised for their fairly low sensitivity. For this reason attention has been given to alternative tests. The aim of this paper is to review the evidence for screening for colorectal cancer using qualitative immunochemical faecal occult blood tests. For the complete range of tested cut-off values, immunochemical faecal occult blood tests lead to higher diagnostic yield, improved sensitivity and greater participation. The optimal number of samples and the optimal cut-off value has to suit local resour…

Transpupillary thermotherapy (TTT) for age-related macular degeneration (AMD) – Is it time to say good-bye?

2010

Abstract Purpose To evaluate the effectiveness of transpupillary thermotherapy (TTT) for the closure of classic and occult subfoveal choroidal neovascularization (CNV) and to discuss whether TTT is a useful treatment modality for age-related macular degeneration (AMD). Materials and methods The present paper is a retrospective review of the clinical results from a patient group which was treated with TTT in the University Eye Clinic in Mainz. From January 2000 to December 2002, a total of 20 eyes of 20 patients with subfoveal CNV (due to AMD in 16 eyes, due to pathologic myopia in 4 eyes) received TTT. The procedure was delivered using an infrared diode laser at 810 nm, equipped with a modi…

Colorectal Polyps

2008

Publisher Summary Colorectal cancer (CRC) remains the second leading cause of cancer death for both women and men with more than 130,000 newly diagnosed cases and 50,000 deaths each year in the United States alone. Most colon cancers develop from nonmalignant colonic adenomas or polyps over a comparatively long time period ranging between 24 and 60 months. Reflecting this adenomatous pathogenesis of most colorectal cancers, polyp screening with subsequent polypectomy is used to constitute an effective approach for decreasing the incidence of this malignant tumor. Thus, colorectal screening for polyps may be considered one of the most promising preventive measures in medicine. Most available…

Der hyperplastische Polyp des Magens - eine Präkanzerose?

2002

In general, hyperplastic polyps of the stomach are regarded as benign lesions. Still there is the chance to find carcinomas within them as documented in this case report. This finding can be interpreted as an incidental appearance. At the moment there are no standardized guidelines regarding diagnostics and therapy of gastric hyperplastic polyps. Referring to a short review of the literature endoscopic snare polypectomy seems to be a reasonable therapeutic option of hyperplastic polyps with a size of 0.5-1 cm. Although it is possible to determine the histology by forceps biopsy, it is possible to be misled by missing areas of focal carcinomas. Complications after polypectomy are rare and of…

"Mroczna otchłań". Archetypowa interpretacja opowiadania "Przemiana Juana Romero" Howarda P. Lovecrafta

2020

The setting of “The Transition of Juan Romero” – the short story by Howard Phillips Lovecraft, who was the originator of modern American horror fiction – is “the gold mine in the Cactus Range”. Drawing on the theories of Carl Gustav Jung, the author of the present interpretation amplifies the cultural dimensions of Lovecraft’s text, giving prominence to its origin from the ancient myths of the Aztecs, and using the so-called psychological alchemy to show the archetypal symbolism and structure of the story. Such a critical perspective, which takes into account different layers of the authorial intentionality and the reader’s response, reveals the complexity of the characters and situations, …

Aseemista okkultismia: Austin Osman Sparen maagiset monogrammit

2018

1800-luvun lopun ja 1900-luvun alun Lontoossa Spareen puolestaan vaikuttivat uudet okkultistiset liikkeet, kuten Hermetic Order of the Golden Dawn, Ordo Templi Orientis ja Aleister Crowleyn (1875–1947) perustama Argentium Astrum. Myöhemmin Sparen tekniikkaa on hyödynnetty erityisesti 1970-luvulla luodun …
