Migration of a Fractured Acetabular Socket Wire Into the Femoral Vein: Indication for Urgent Removal by Venous Thrombectomy: A Case Report

The migration of a foreign body into a great vein is a rare occurrence that requires urgent surgical intervention. In our patient one fragment of a circular acetabular socket wire marker had entered the femoral vein. Exact topographic location of the wire was achieved by computerized tomography. Urgent surgery revealed a floating wire within the venous lumen with associated thrombosis.

Laparoscopic Treatment of Intussusception

Most cases of ileocolic intussusception are reduced by imaging-guided air or fluid enema. If unsuccessful, operative intervention is necessary. The laparoscopic approach allows inspection of the bowel, identification of intraabdominal pathology, and achieves reduction of the intussuscepted bowel in most cases. The procedure requires the combination of slow and gradual traction as well as “rolling” the cecum distally. If unsuccessful, the bowel can be delivered through an extended umbilical incision for extracorporeal manipulation, or resection if this should be necessary.

Skin-reducing Mastectomy with Primary Implant Reconstruction

Background: We present a series of skin-sparing mastectomies (SSMs) with skin reduction and immediate breast reconstruction to treat large and ptotic breasts. The technique combines oncological mastectomy with immediate subpectoral implant placement as a single-step procedure. Methods: Data was collected from a prospective database from February 2009 to April 2011. A total of 24 patients with macromastia or pronounced ptosis fulfilled the criteria for skin-saving mastectomy. Osteoporosis is “a systemic disease characterized by a reduction in bone mass and a microarchitectonic alteration of the skeleton, with a consequent increase in skeleton fragility and susceptibility to fractures”, as defined by the last Consensus Development Conference1. Abstract Introduction Positive airway pressure (PAP) treatment modifies blood pressure (BP) in patients with obstructive sleep apnea (OSA). We aimed to explore which factors that influence the BP response to PAP therapy in the European Sleep Apnea Database (ESADA). Methods A total of 2662 OSA patients with PAP therapy ≥90 days were included in the analysis (74% male, age 55±11 years, body mass index 32.3±6.1 kg/m2, 47% hypertensives, apnea-hypopnea index 40±24 events/h, treatment duration 1.0±1.1 years, PAP compliance 5.2±1.9 h/day). In patients over 65y.o. who were surgically treated for a hip fracture, electrolytes have not been specifically studied as predictors of mortality. The main purpose of this study was to assess whether electrolytes and chronic kidney disease (CKD) stages, evaluated at admission, could represent a pre-operative prognostic factor in this population.
