Osteoporosis: Economic Burden of Disease in Italy.

2020

Background and Objective: Today, osteoporosis is the most common bone disease and an important public health problem in all developed countries. The objective of this study was to estimate the costs associated with the management and treatment of osteoporosis in order to assess the economic burden in Italy for 2017, in terms of direct medical costs and social security costs. Methods: A cost of illness model was developed to estimate the average cost per year sustained by the NHS (National Health Service) and Social Security System in Italy. A systematic literature review was performed to obtain epidemiological, direct and indirect costs parameters where available. Hospitalisation costs were…

Concerted actions of insulin-like growth factor 1, testosterone, and estradiol on peripubertal bone growth: a 7-year longitudinal study.

2011

A better understanding of how bone growth is regulated during peripuberty is important for optimizing the attainment of peak bone mass and for the prevention of osteoporosis in later life. In this report we used hierarchical models to evaluate the associations of insulin-like growth factor 1 (IGF-1), estradiol (E(2) ), and testosterone (T) with peripubertal bone growth in a 7-year longitudinal study. Two-hundred and fifty-eight healthy girls were assessed at baseline (mean age 11.2 years) and at 1, 2, 3.5, and 7 years. Serum concentrations of IGF-1, E(2) , and T were determined. Musculoskeletal properties in the left lower leg were measured using peripheral quantitative computed tomography …

Vitamin D status as a determinant of peak bone mass in young Finnish men.

2004

Severe vitamin D deficiency causes rickets, but scarce data are available about the extent to which vitamin D status determines the development of the peak bone mass in young adults. Our aim was to evaluate the prevalence of vitamin D deficiency [serum 25-hydroxyvitamin D (25-OHD) less than the lower limit of the reference range of 20–105 nmol/liter] and the relationship between vitamin D status and peak bone mass among young Finnish men. A cross-sectional study of determinants of peak bone mass with data on lifestyle factors collected retrospectively was performed in 220 young men, aged 18.3–20.6 yr. One hundred and seventy men were recruits of the Finnish Army, and 50 were men of similar …

Muscle training for bone strength

2006

The main function of bone is to provide the mechanical integrity for locomotion and protection; accordingly, bone mass and architecture are adjusted to control the strains produced by mechanical load and muscular activity. Age-related patterns involve peak bone mass during growth, a plateau in adulthood, and bone loss during aging. The decline in bone mass and structural integrity results in increased risk of fractures, particularly in post-menopausal women. Athletes competing in strength and power events, such as weight-lifting and jumping, have superior bone mass and structure compared with their untrained counterparts in all age groups. Exercise seems to be most effective during rapid gr…

Physical Activity and Exercise in the Maintenance of the Adult Skeleton and the Prevention of Osteoporotic Fractures

2013

Abstract Physical activity and exercise are promoted as a nonpharmacological strategy to maintain or enhance musculoskeletal health in adults and prevent osteoporotic fracture. Less clear is the optimal mode, intensity, duration, and frequency of activity to achieve this objective. Although a substantial osteogenic response to appropriate mechanical loading is clearly evident in animal models and strongly supported from athlete cross-sectional and prospective studies, the results from exercise interventions in adults are less impressive with modest net benefits generally of 1% to 3% in bone mineral density (BMD) (g/cm2) at the clinically relevant hip and spine following prolonged weight-bea…

Epigenetics in spine curvature disorders

2022

Abstract Scoliosis is a three-dimensional (3D) structural deformity of the spine with a radiological lateral Cobb angle of ≥ 10°. Several classification systems exist, dividing different types regarding the age of onset or the type of etiology. The minority of cases are secondary to congenital, syndromic of neuromosucular diseases. Most of the cases are classified “idiopathic” due to unknown etiology. These were formally divided by the age of onset into “Infantile Idiopathic Scoliosis” (0–3 years), “Juvenile Idiopathic Scoliosis” (JIS—4–10 years), and “Adolescent Idiopathic Scoliosis” (AIS → 10 years). Since the initiative of the Scoliosis Research Society in 2014 all kind of scoliosis with…

Osteodystrophy in chronic liver diseases

2013

Osteoporosis and osteomalacia are, to date, among the most common metabolic diseases in the world. Lately, an association between metabolic bone diseases and chronic liver disease has been increasingly reported, inducing many authors to create a new nosographic entity known as 'hepatic osteodystrophy.' The importance of such a condition is further increased by the morbidity of these two diseases, which greatly reduce the quality of life because of frequent fractures, especially vertebral and femoral neck ones. For this reason, early identification of high-risk patients should be routinely performed by measuring bone mass density. The explanation for the association between bone diseases and…

Rehabilitation in a case of severe osteoporosis with prevalent fractures in a patient known with multiple sclerosis and prolonged glucocorticoid ther…

2021

Introduction. Osteoporosis has a major influence on the quality of life because of its impact on bone strength. Osteoporosis and fractures are frequent in patients with multiple sclerosis, decreased mobility being an important risk factor in these patients. Objectives. This paper presents a case of severe osteoporosis in a patient with multiple sclerosis, to emphasize a correlation between this two pathologies. Material and Methods. We present the case of a female Caucasian patient, aged 65 years, known with progressive multiple sclerosis, on long-term use of glucocorticoids, and severe osteoporosis, who is investigated for mechanical pain and functional deficiency in the lumbar spine and t…

Dental considerations in pregnancy and menopause

2011

The present study offers a literature review of the main oral complications observed in women during pregnancy and menopause, and describes the different dental management protocols used during these periods and during lactation, according to the scientific literature. To this effect, a PubMed-Medline search was made, using the following key word combinations: “pregnant and dentistry”, “lactation and dentistry”, “postmenopausal and dentistry”, “menopausal and dentistry” and “oral bisphosphonates and dentistry”. The search was limited to reviews, metaanalyses and clinical guides in dental journals published over the last 10 years in English and Spanish. A total of 38 publications were evalua…

Osteoporosis risk factors in HIV positive women with osteoporosis: A retrospective analysis

2017

Multifactorial risk factors such as HIV/HCV co-infection and antiretroviral therapy (ARV) have been associated with osteoporosis in HIV+ women. We retrospectively analysed which known risk factors were associated with the diagnosis of osteoporosis, according to the WHO definition, in HIV positive women who were followed-up at the AIDS Centre of the University of Palermo, Italy between January 2011 and December 2014. Twenty-one HIV+ women with osteoporosis (13 HIV+ mono-infected and 8 HIV/HCV co-infected females) who underwent dual-energy X-ray absorptiometry (DXA) and liver stiffness assessment were included in the study. No significant differences between the HIV and HIV/HCV group were fou…
