The differential diagnosis of vasculitis is often a difficult task due to the frequent morphological similarities that various vasculitic syndromes express when the heart is the target organ. The more the lesions are limited to the arterial tree with absent or almost silent coronary events, the less specific the anatomical and clinical frameworks. Aims: We aimed to investigate the outcomes of bioresorbable vascular scaffolds (BVS) in coronary ostial lesions. Ostial lesions represent a challenging angiographic subset, with higher event rates compared with non-ostial lesions. Coronary artery anomalies are a heterogeneous group of congenital disorders affecting the coronary pattern with a characteristic (origin, number, number of hosts, course, etc.). They are rarely found in the population. We report the case of a 21-year-old male who died from sudden cardiac death caused by an isolated coronary ostial obstruction, secondary to a rare anomaly, the presence of a valve-like ridge consisting of a small fold in the Valsalva aortic sinus. La diagnosi differenziale nelle vasculiti è spesso impegnativa per le frequenti similitudini morfologiche che diverse sindromi vasculitiche esprimono quando l’organo bersaglio è il cuore. I quadri anatomo-clinici sono sempre meno specifici quanto più le lesioni sono confinate esclusivamente all’albero coronarico con manifestazioni extracardiache assenti o pressochè silenti.
