This review article discusses the medical need for improved medical treatments of overactive bladder syndrome, and the hurdles and research required to address that need. Currently, few overactive bladder syndrome patients stay on long-term treatment, largely because efficacy expectations are not met, and tolerability is considered insufficient for the chronic treatment of a non-life-threatening condition. Therefore, a medical need exists for improved tolerability and, even more importantly, improved efficacy. Abstract Overactive bladder (OAB) is the term introduced by the International Continence Society in 2002 to describe the symptoms of urgency, with or without urge incontinence, usually in the presence of frequency and nocturia. Community-based prevalence studies have shown an estimated 16% of the adult population in the US and Europe suffers the symptoms of OAB, and in many cases the symptoms can have a profound effect on quality of life.
