Excited-state absorption in erbium-doped silica fiber with simultaneous excitation at 977 and 1531 nm

2009

We report a study of the excited-state absorption (ESA) in erbium-doped silica fiber (EDF) pumped at 977 nm, when the fiber is simultaneously excited by signal radiation at 1531 nm. We show, both experimentally and theoretically, that ESA efficiency at 977 nm gets strongly enhanced only in the presence of signal power. Experimentally, this conclusion is supported through the detection of upconversion emission, a “fingerprint” of the ESA process, and through the measurements of the EDF nonlinear transmission coefficient for the pump wavelength, which is sensitive to the ESA value. It is shown that the experimental data are precisely modeled with an advanced five-level Er3+ model developed fo…

The Androgen Excess and PCOS Society criteria for the polycystic ovary syndrome: the complete task force report.

2009

Abstract OBJECTIVE: To review all available data and recommend a definition for polycystic ovary syndrome (PCOS) based on published peer-reviewed data, whether already in use or not, to guide clinical diagnosis and future research. DESIGN: Literature review and expert consensus. SETTING: Professional society. PATIENTS: None. INTERVENTION(S): None. MAIN OUTCOME MEASURE(S): A systematic review of the published peer-reviewed medical literature, by querying MEDLINE databases, to identify studies evaluating the epidemiology or phenotypic aspects of PCOS. RESULT(S): The Task Force drafted the initial report, following a consensus process via electronic communication, which was then reviewed and c…

Poor diagnostic value of the euglycemic-hyperinsulinemic clamp in the clinical assessment of insulin resistant women with PCOS.

2013

Insulin resistance (IR) per se and/or the ensuing compensatory hyperinsulinism, have been always considered as a key point in the pathogenesis of PCOS. Aim of our study was to evaluate which of these two aspects plays a key role in the pathogenesis of PCOS. Using a cross-sectional study design, 15 PCOS (Rotterdam criteria) and 16 age/BMI-matched control women (with suspect of IR) underwent clinical measures of IR after a 3-month withdrawal of insulin sensitizers and oral contraceptive pills. In an academic clinic setting, average glucose infusion rate (M-value) (during an euglycaemic–hyperinsulinaemic clamp), AUCinsulin and AUCglucose (during an Oral Glucose tolerance Test), HOMA-IR, ISI-Ma…

Estroprogestins and cardiovascular risk in Polycystic Ovary Syndrome

2013

Because PCOS women present an increased cardiovascular risk, the safety of estroprogestin treatment is debated and contrasting data have been reported. However, cardiovascular risk is not the same in all PCOS women and individual cardiovascular risk should be assessed before staring any estroprogestin treatment. The available data show that products containing both second generation and third generation progestins (including drosperinone and cyproterone acetate) represent a safe treatment in PCOS patients with regular cardiovascular risk. In PCOS patients with increased cardiovascular risk, a careful choice of estroprogestin product is needed and cardiovascular risk should be monitored duri…

Impact of obesity, insulin resistance and hyperandrogenism on steatosis and fibrosis risk in young females with PCOS

2017

Obesity and insulin resistance (IR) represent a common pathogenic background of polycystic ovary syndrome (PCOS) and nonalcoholic fatty liver disease (NAFLD). We evaluated whether PCOS is a risk factor for steatosis, and subsequently, the impact of IR and hyperandrogenism on steatosis and fibrosis. We considered 202 consecutive non diabetic PCOS patients and 101 age-matched controls. PCOS was diagnosed applying the Rotterdam diagnostic criteria. Steatosis was diagnosed if hepatic steatosis index (HSI) >36, while fibrosis by using the FIB-4 score. As surrogate estimate of insulin sensitivity we considered the insulin sensitivity index (ISI). Free androgen index (FAI) was calculated as estima…
