Protective ventilation with high versus low positive end-expiratory pressure during one-lung ventilation for thoracic surgery (PROTHOR): study protoc…
2019
Background: Postoperative pulmonary complications (PPC) may result in longer duration of in-hospital stay and even mortality. Both thoracic surgery and intraoperative mechanical ventilation settings add considerably to the risk of PPC. It is unclear if one-lung ventilation (OLV) for thoracic surgery with a strategy of intraoperative high positive end-expiratory pressure (PEEP) and recruitment maneuvers (RM) reduces PPC, compared to low PEEP without RM. Methods: PROTHOR is an international, multicenter, randomized, controlled, assessor-blinded, two-arm trial initiated by investigators of the PROtective VEntilation NETwork. In total, 2378 patients will be randomly assigned to one of two diffe…
Realization of polyaspartamide-based nanoparticles and in vivo lung biodistribution evaluation of a loaded gucocorticoid after aerosolization in mice
2016
Abstract In this study, novel polymeric nanoparticles (NPs) were developed and their potential as carriers for beclomethasone dipropionate (BDP) into the lung after aerosolization was demonstrated by in vivo studies in mice. In particular, these NPs were obtained starting from two polyaspartamide-based copolymers which were synthesized by chemical reaction of α,β-poly(N-2-hydroxyethyl)- dl -aspartamide (PHEA) and its pegylated derivative (PHEA-PEG2000) with poly(lactic acid) (PLA). To obtain nanosized particles, the high pressure homogenization (HPH)—solvent evaporation method was followed by using an organic phase containing both PHEA-PLA and PHEA-PEG2000-PLA (at a weight ratio equal to 1:…
Pirmreizējas sirds ritma izmaiņas pacientiem pēc atvērtās sirds operācijas
2019
CD3+ and CD8+ T-Cell-Based Immune Cell Score and PD-(L)1 Expression in Pulmonary Metastases of Microsatellite Stable Colorectal Cancer
2022
Simple Summary The lung is the second most common site of metastases in colorectal cancer (CRC). The aim of our study was to evaluate the prognostic value of CD3+ and CD8+ T-cell density based immune cell score (ICS) and PD-1/PD-L1 expression in resected pulmonary metastases of microsatellite stable CRC. The T-cell infiltration was higher in the first pulmonary metastases compared to primary tumour. Pulmonary metastases with high ICS had improved survival compared to low ICS after adjusting for confounders. High tumour cell PD-L1 expression was associated with favourable prognosis. Our results might have clinical feasibility in planning future therapies. Abstract The objective of this study…
Remodeling of bronchial and pulmonary muscular arteries in mild/moderate COPD
2014
Pulmonary hypertension is well documented in severe COPD but little is known about vascular remodeling in mild/moderate patients. Aim:To investigate bronchial and pulmonary muscular arteries morphology and the expression of elastic fibers (EF) and collagen I in the lungs of mild/moderate COPD patients. Methods:Lung tissue of 18 COPD subject (FEV1:67±15%pred), 14 non-obstructed smokers(NOS) (FEV1:95±10%pred) and 11 never-smoking controls(CTRL) (FEV1:108±15%pred) was used to analyze the morphology of bronchial and pulmonary muscular arteries and the expression of EF and collagen I in the tunica intima, media and adventitia, using immune- and histochemistry and image analysis. Values (mean±SD)…
Physicochemical investigation of surfactant-coated gold nanoparticles synthesized in the confined space of dry reversed micelles
2006
Abstract Gold nanoparticle/surfactant composites have been synthesized by a novel reduction reaction in the confined space of dry sodium bis(2- ethylhexyl)sulfosuccinate (AOT) or lecithin reversed micelles dispersed in n-heptane and cyclohexane, respectively. The reaction was carried out by adding an opportune amount of anhydrous hydrazine/tetrahydrofuran solution to a suspension of HAuCl4-containing dry reversed micelles dispersed in organic solvent. UV–vis investigation ascertained the formation of stable metal gold nanoparticles and the analysis of FT-IR spectra highlighted the formation of an oriented surfactant monolayer at the nanoparticle surface. Simple evaporation under vacuum of t…
Intravenous Contrast Material for Cardiac Computed Tomography
2023
Purpose: The Italian Registry of Contrast Material use in Cardiac Computed Tomography (iRCM-CCT) is a multicenter, multivendor, observational study on the use of contrast media (CM) in patients undergoing cardiac computed tomography (CCT). The aim of iRCM-CCT is to assess image quality and safety profile of intravenous CM compounds. Materials and methods: iRCM-CCT enrolled 1842 consecutive patients undergoing CCT (≥50 per site) at 20 cluster sites with the indication of suspected coronary artery disease. Demographic characteristics, CCT, and CM protocols, clinical indications, safety markers, radiation dose reports, qualitative (ie, poor vascular enhancement) and quantitative (ie, HU attenu…
Effects of hyperoxemia in patients with sepsis – A post-hoc analysis of a multicentre randomized clinical trial
2023
Background Administration of supplemental oxygen is a life-saving treatment in critically ill patients. Still, optimal dosing remains unclear during sepsis. The aim of this post-hoc analysis was to assess the association between hyperoxemia and 90-day mortality in a large cohort of septic patients. Methods This is a post-hoc analysis of the Albumin Italian Outcome Sepsis (ALBIOS) randomized controlled trial (RCT). Patients with sepsis who survived the first 48 h since randomization were included and stratified into two groups according to their average PaO2 levels during the first 48 h (PaO2 0–48 h). The cut-off value was established at 100 mmHg (average PaO2 0–48 h >100 mmHg: hyperoxemi…
Lemierre Syndrome associated with dental infections : report of one case and review of the literature
2007
The first publication on Lemierre Syndrome appears in 1936 by Lemierre. It is defined as an ?oropharynx bacterial infection characterized by the thrombophlebitis in the internal jugular vein, derived in a systemic septic embolism?. In 81% of the cases, the Fusobacterium necrophorum is the most frequent etiologic agent. Fever is the most common symptom, but it can depending on the primary infection, tonsillitis, mastoiditis or odontogenic infection. According to the literature the mortality is very low, but with a significant morbidity, that is why the diagnosis and early treatment is very important. The diagnosis it´s clinical, even though the CT scan and other diagnosis methods (echography…
