A new method to value efficiency of enzyme blends for pancreatic tissue digestion.

2010

Islet transplantation, since the 90’s, has been resulting to be one of the best successful example of human cell therapy. Nevertheless, islet isolation procedure is not completely standardized; in fact, more than fifty percent of islets procedures don’t arrive to their transplantation. This is due both to the variability of donor’s pancreas and to an unpredictable enzymatic blend efficiency. Enzymes used in pancreas digestion are extracted from Clostridium histolyticum bacteria and digest several substrates. In particular they have strong collagenolytic activity compared to vertebrate collagenases. However, several impediments persist in human islet isolation success probably due to the var…

Protection of islets in culture by delivery of oxygen binding neuroglobin via protein transduction.

2005

Islet transplantation has become an accepted method to treat type 1 diabetes. To succeed and achieve normal levels of glucose in transplant recipients, the quality of the transplanted islets is of the utmost importance. Lack of oxygen during organ procurement, islet isolation, and subsequent culture triggers apoptosis or necrosis and loss of islet function, causing the yield and quality to diminish. A promising candidate for cytoprotection against oxygen deprivation is neuroglobin (Ngb). Ngb is a recently described member of globin family and is expressed in neurons, retina, and pancreatic islets. To overexpress this protein in the islets and study its ability to protect them, we utilized p…

Impact of biliary stents on the diagnostic accuracy of EUS-guided fine-needle biopsy of solid pancreatic head lesions: A multicenter study

2021

Background and Objectives: There is no clear evidence of a negative impact of biliary stents on the diagnostic yield of EUS-guided fine-needle biopsy (EUS-FNB) for diagnosing pancreatic head lesions. We aimed to evaluate the association between the presence of biliary stents and the diagnostic accuracy of EUS-FNB. Materials and Methods: A multicenter retrospective study including all jaundiced patients secondary to pancreatic head masses was performed. Patients were divided into two groups according to the presence of a biliary stent placed before EUS-FNB. Pathological results were classified according to the Papanicolaou classification and compared against the final diagnosis. Diagnostic m…

Multi-Criteria Decision Making support system for pancreatic islet transplantation

2011

Pancreatic islet transplantation consists of replacing insulin-producing cells to restore normal glycemia in diabetic patients. This is a minimal invasive procedure that has been proved successful. Unfortunately unpredictability of islet transplant outcome remains a frustrating and costly issue limiting the clinical implementation of this procedure. Multiple variables are involved in the procedure and assessment is subjective to individual operators. The aim of this study was to generate a system expressing the probability of transplant success in relation to four classes of identified variables (donor, organ, isolation and recipient). We have proposed the utilization of Multi-Criteria Deci…

Cell Therapy in Type 1 Diabetes

2011

The incidence of diabetes mellitus has grown exponentially in the last few years. Etiopathogenesis of diabetes implies a ?-cells damage in the islet of Langerhans, either through an autoimmune reaction present in type 1 diabetic patients or through altered function within these cells that affect their ability to secrete a properly functioning insulin hormone, in patients suffering from type 2 diabetes. Exogenous insulin supply is, at the moment, the therapy of choice of the disease but it does not allow tight control of glucose regulation, leading to long-term complications. Over the past few decades, pancreas or pancreas-kidney organ transplantation has been the most effective treatment fo…

Quality of life of patients with advanced pancreatic cancer

2021

Pancreatic cancer is one of the most common malignancies with poor prognosis and high mortality. Advanced-stage disease at diagnosis and the dominant clinical symptoms significantly deteriorate the quality of life. The paper presents an analysis of the results of quality of life studies in patients with locally advanced and metastatic pancreatic cancer, as well as the relationship between therapeutic decisions and quality of life indicators. It has been shown that the initial assessment of life quality can have prognostic value.Appropriate symptomatic treatment of patients with advanced pancreatic cancer improves the quality of life, increases the compliance and prolongs survival. The asses…

Role of ghrelin and leptin in the regulation of carbohydrate metabolism. Part I. Ghrelin

2012

The microbiota of the bilio-pancreatic system: a cohort, STROBE-compliant study.

2019

Background: The gut microbiota play an essential role in protecting the host against pathogenic microorganisms by modulating immunity and regulating metabolic processes. In response to environmental factors, microbes can hugely alter their metabolism. These factors can substantially impact the host and have potential pathologic implications.  Particularly pathogenic microorganisms colonizing pancreas and biliary tract tissues may be involved in chronic inflammation and cancer evolution. Purpose: To evaluate the effect of bile microbiota on survival in patients with pancreas and biliary tract disease (PBD). Patients and Methods: We investigated 152 Italian patients with cholelithiasis (CHL),…
