Host defence mechanisms against bacterial aggression in periodontal disease : basic mechanisms

2009

Periodontal diseases are complex bacteria-induced infections characterised by an inflammatory host response to plaque microbiota and their by-products. Most of these microorganisms have virulence factors capable of causing massive tissue destruction both directly, through tissue invasion and the production of harmful substances, or indirectly, by activation of host defense mechanisms, creating an inflammatory infiltrate of potent catabolic activity that can interfere with normal host defense mechanisms. In response to the aggression, host defense mechanisms activate innate and adaptive immune responses. Our aim is to offer a general overview of the main mechanisms involved in the host respo…

Non-surgical management of chronic periodontitis with two local drug delivery agents-A comparative study

2011

Background: The selective removal or inhibition of pathogenic microbes with locally delivered antimicrobials when combined with scaling and root planing is often an effective approach for the managment of chronic periodontitis. Aim: To compare the clinical efficacy of tetracycline fibers and a xanthan based chlorhexidine gel in the treatment of chronic periodontitis. Methods and materials: Thirty systemically healthy patients in the age group of 30-50 years suffering from generalized chronic moderate periodontitis were selected. For each subject, two experimental sites were chosen that had probing depth >5mm and were located in symmetric quadrants and the sites were randomized at split mout…

Biofilm and Orthodontic Therapy

2020

Dental biofilms can cause major oral diseases like gingivitis, periodontitis, and caries. Orthodontic appliances promote supra- and subgingival biofilm accumulation, alter the oral microbiome, and hamper oral hygiene. Orthodontic treatment can be associated with adverse effects, such as enamel decalcification, gingivitis, and periodontal disease. The aim of this review is to summarize the changes in supra- and subgingival biofilm and periodontal tissues during and after orthodontic treatment. Studies have reported elevated levels of Streptococcus mutans and periodontopathogenic bacteria in patients undergoing orthodontic treatment. In general, the microbial changes and periodontal parameter…

Oral microcirculation in post-menopause: a possible correlation with periodontitis

2011

doi: 10.1111/j.1741-2358.2011.00608.x Oral microcirculation in post-menopause: a possible correlation with periodontitis Objectives:  The reduction in the level of oestrogen, typical in menopause, has some effect on the health of the oral cavity. In fact, post-menopausal women present more severe periodontal disease than pre-menopausal women. Numerous factors can be held to be responsible for this increase, among which are the effects of oestrogens on the oral epithelium, on the salivary glands, on bone tissue and on the endothelium. Our double blind study aims to evaluate the possible variations in oral microcirculation in post-menopausal women. Methods:  Twenty-seven women in post-menopau…

MicroRNAs and periodontal disease: a qualitative systematic review of human studies.

2020

Purpose MicroRNAs (miRNAs) are epigenetic post-transcriptional regulators that modulate gene expression and have been identified as biomarkers for several diseases, including cancer. This study aimed to systematically review the relationship between miRNAs and periodontal disease in humans, and to evaluate the potential of miRNAs as diagnostic and prognostic biomarkers of disease. Methods The review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines (reference number CRD42020180683). The MEDLINE, Scopus, Cochrane Library, Embase, Web of Science, and SciELO databases were searched for clinical studies conducted in humans investigating pe…

Oral lesions and immune status of HIV infected adults from eastern Nepal

2013

Objective: To document the prevalence, age and gender distribution of oral lesions in HIV infected adults and the influence of highly active antiretroviral therapy and correlate them to the immune status of the patients. Materials and Methods: Oral lesions were diagnosed by a detailed physical examination by trained and calibrated examiners according to the case definitions established by the Oral HIV/AIDS research alliance. Demographic details, risk behavior patterns and oral symptoms and habits were collected by a questionnaire. Results: 81 patients; 54 men and 27 women aged between 20 ' 55 years participated in the study. A total of 49 patients; 60.5% had some oral lesion when examined. …

Necrotizing periodontitis in the context of Takayasu s disease: report of a case

2011

Necrotizing periodontitis is characterized by gingival necrosis with bleeding and pain, and alveolar bone attachment loss. The disease is associated to spirochete and Prevotella intermedia infection, as well as to a series of bacterial infections, with an altered local host immune response and tissue destruction. We present the case of a 40-year-old woman with inflammatory vasculitis of the aorta and its main branches, known as Takayasu’s disease. In the course of treatment of the latter, she developed necrotizing periodontitis. In this case local factors such as abundant bacterial plaque accumulation secondary to a lack of tooth brushing, and the administration of methotrexate, may have pl…

Papillon- Lefevre Syndrome: Report of a case and its management

2012

Papillon-Lefèvre Syndrome (PLS) is a rare autosomal recessive disorder first described by two French physicians, Papillon and Lefèvre in 1924. The disorder is characterized by diffuse palmoplantar keratoderma and precocious aggressively progressing periodontitis, leading to the premature loss of deciduous and permanent teeth at a very young age. The cutaneous lesions are usually manifested simultaneously with the intra-oral presentations and include keratotic plaques on the palms and soles varying from mild psoriasiform scaly skin to overt hyperkeratosis. The etiopathogenesis of the syndrome is relatively obscure and immunologic, genetic or possible bacterial etiologies have been proposed. …

Dental considerations in pregnancy and menopause

2011

The present study offers a literature review of the main oral complications observed in women during pregnancy and menopause, and describes the different dental management protocols used during these periods and during lactation, according to the scientific literature. To this effect, a PubMed-Medline search was made, using the following key word combinations: “pregnant and dentistry”, “lactation and dentistry”, “postmenopausal and dentistry”, “menopausal and dentistry” and “oral bisphosphonates and dentistry”. The search was limited to reviews, metaanalyses and clinical guides in dental journals published over the last 10 years in English and Spanish. A total of 38 publications were evalua…
