A breathing-retraining procedure in treatment of sleep-onset insomnia: theoretical basis and experimental findings.

Increase in CO2 has a sedative effect upon the central nervous system, and the beginning of sleep coincides with modifications in breathing, decrease in ventilation, and in pCO2 increase. In this paper is described a technique of breathing that is useful in producing drowsiness in a very short time. 46 insomniacs were randomly allocated to either a treatment or control condition. In the former, patients were trained in the breathing process. The control group was taught no breathing process. Latencies to sleep for the insomniacs confirmed that the breathing process was useful in producing drowsiness. Theoretical bases are discussed.
Hemisensory disturbances in patients with complex regional pain syndrome.

Sensory disturbances often spread beyond the site of injury in complex regional pain syndrome (CRPS) but whether this applies equally to CRPS I and II, or changes across the course of the disease, is unknown. Establishing this is important, because different symptom profiles in CRPS I and II, or in acute vs chronic CRPS, might infer different pathophysiology and treatment approaches. To explore these questions, sensory disturbances were assessed in the limbs and forehead of 71 patients with CRPS I and 33 patients with CRPS II. The musculoskeletal capacity of 60 women and 69 men, average age 52.3 +/- 3.7 years was determined, including measurements of anthropometry, maximal isometric trunk flexion and extension, sit-ups, isometric hand grip strength and back mobility. According to the job and to cluster analysis, the subjects were divided into three dominating work groups; physical, mental, and mixed groups. The results showed significant differences in right hand grip strength of the women and in the number of sit-ups by men among the three work groups (p less than 0.05). We investigated muscle strength, aerobic power, and occupational and leisure-time physical loading as predictors of back pain in a 5-year follow-up study. A cohort of 456 adults aged 25, 35, 45 and 55 years, free of back pain, participated in measurements of anthropometric characteristics, aerobic power and muscle strength characteristics at baseline. The subjects' levels and types of physical activity and occupational physical loading were also determined. The perceived exertion rating (RPE) scale of Borg was used to investigate the relationship between perceived exertion and pedalling rate. Normal subjects and patients with chronic obstructive lung disease (Cold) were studied in repeated test series. Work load, applied in a random order, varied from 2.5 to 10 mkp/s (patients) and 5 to 20 mkp/s (normals). Pedalling rate varied from 2.5 to 10 mkp/s (patients) and 5 to 20 mkp/s (normals). The purpose of this study was to evaluate the effects of 21 weeks of strength and/or endurance training on health related quality of life (HRQoL) in 39-77 year-old healthy subjects. 108 men and 96 women were randomized into endurance, strength, or combined training groups and controls. Strength-group performed high-intensity strength training while endurance-group performed cycle training. Combined-group completed both training protocols. Leg extension strength and maximal oxygen uptake were measured. HRQoL was assessed with a Finnish version of SF-36 questionnaire.
