In addition to being a major cause of morbidity and mortality, hypertension places a heavy burden on health care systems, families, and society as a whole. Despite evidence of an increasing prevalence of hypertension among youth, the consequences of early onset are poorly established and often overlooked. Childhood hypertension is often asymptomatic and easily missed, even by health professionals. Obesity is a common disease with an ever-increasing prevalence and usually with late-onset consequences. If acquired during childhood, it tracks into adult life to some extent, and since the relationship between obesity and hypertension is well established in adults, obese children appear to be at particularly high risk of becoming hypertensive adults. IntroductionFor Major Depression, current guidelines recommend treatment durations of 3-8 weeks until optimisation in case of insufficient outcome. Many retrospective studies indicate that improvement (HAMD-17 decrease ≥20%) occurs usually within 10-14 days and that non-improvement after 14 days of treatment is highly predictive for poor clinical outcome.MethodsIn level 1 of the EMC trial, non-improvers after 14 days of antidepressant treatment will be randomised to “early medication change” (EMC) strategy or treatment according to current guidelines (TAU). Abstract The occlusion of two adjacent cortical veins is followed by a widespread reduction of rCBF and the occurrence of small infarcts, which become larger if spreading depression (SD) occurs. The model appears to be useful for studying penumbra pathophysiology. Here, the time course of gene expression in the penumbra was compared to those induced by the SD alone. In an experiment involving rats, an occlusion of two cortical veins was induced by i.v. rose bengal and fiberoptic illumination. Ten SDs were induced at 7-min intervals.
