Cost-effectiveness analysis of statins in primary care: results from the Arteris cohort study.
2021
BACKGROUND The Swiss Federal Office of Public Health performed a health technology assessment regarding statins in primary care. The chosen models may lead to a situation where a clinically indicated statin therapy is estimated not to be cost effective. METHODS We performed a cohort study regarding cardiovascular events, comparing SCORE and AGLA risk categories with tertiles of carotid plaque burden and used two models for cost-effectiveness analysis of high-potency statins. RESULTS Subjects (n = 2842) were followed up for 5.9 ± 2.9 years with the occurrence of 154 cardiovascular events (extrapolated 10-year risk was 9.2%). Carotid plaque imaging (total plaque area, TPA) significantly impro…
Control de la dislipemia en grupos clínicos especiales: mujeres, ancianos, VIH, enfermedad renal crónica, pacientes con síndrome metabólico
2011
There is still some disagreement about how dyslipidemia should be treated in particular population groups. This article provides a review of indications for lipid-lowering treatment, preferably for primary prevention, in specific groups of patients: women, the elderly, and those with an HIV infection, chronic kidney disease or metabolic syndrome. Special emphasis is placed on data obtained in the JUPITER trial and in other studies of rosuvastatin.
Assessment of the health care system functioning in Poland in light of the analysis of the indicators of the hospital emergency department (ED) and p…
2019
Background In the last three decades, in developed countries, the number of patients reporting to hospital emergency departments has dramatically increased. The number of visits to EDs can be an important indicator of the quality of primary health care. Objectives To analyse: 1) the frequency of admissions to EDs, 2) the type and number of medical procedures performed in EDs, and 3) the type and number of services provided by PHC in the Kedzierzyn-Kozle district. Material and methods A retrospective analysis of the statistical data regarding services provided by PHC in the Kedzierzyn-Kozle district within the National Health Fund (NHF) and data from the hospital emergency department in Kedz…
Experiences of family physicians after a CME ultrasound course
2017
Background. Ultrasonography is an inexpensive, safe, noninvasive tool for obtaining diagnoses without performing further imaging tests, and for the timely referral of patients to further diagnosis or specialist care. Objectives . To determine which ultrasound tests are most often performed by family physicians, and what factors contribute to their use of ultrasound imaging. Material and methods . This survey-based study using the authors’ questionnaire was conducted in March 2016 among 81 primary care physicians who had completed weekend courses on ultrasound abdominal cavity imaging. Results . Family physicians most often took ultrasounds of the abdominal cavity (35/50; 70%) and thyroid gl…
Endoscopic training during fellowship: A nationwide French study
2022
ABSTRACT Background In France, it is mandatory that gastroenterology fellows have mastered the basic level of endoscopy by the end of training. The aim of this study was to assess improvement in the quality of fellows’ endoscopy training in France during the last four years. Methods All fellows in France in training were eligible for participation. A 21-item questionnaire was sent out. The primary outcome was the completion by fourth year fellows of all the number of procedures recommended. Results were compared with those of a 2016 survey. Results Two-hundred-and-sixty-five fellows responded to the survey. The participation rate was 47.0%. The mean age was 27.3 ± 1.0 years and 56.4% were f…
Validity of the PHQ-9 and PHQ-2 to screen for depression in nationwide primary care population in Latvia
2018
Abstract Background Depression is highly underdiagnosed in primary care settings in Latvia. Screening for depression in primary care is potentially an efficient way to find undetected case s and improve diagnostics. We aimed to validate both a nine-item and two-item Patient Health Questionnaire (PHQ-9 and PHQ-2) in the Latvian and Russian languages in primary care settings using a representative sample in Latvia. Materials and methods The study was carried out within the framework of the National Research Program BIOMEDICINE to assess the prevalence of mental disorders at 24 primary care facilities. During a 1-week period, all consecutive adult patients were invited to complete the PHQ-9 an…
Five Interpersonal Factors Are Predictive of the Response to Treatment of Major Depression With Antidepressants in Primary Care
2018
Introduction: Factors relating to the interpersonal relationship between the patient and their physician and social environment are important components, which contribute to their response to treatment for major depressive disorder. This study aimed to assess the influence of optimism, perfectionism, therapeutic alliance, empathy, social support, and adherence to medication regimen in the response to antidepressant treatments in the context of normal primary care clinical practice.Method: We conducted a prospective study in which 24 primary care physicians administered sertraline or escitalopram to 89 patients diagnosed with major depressive disorder. The response to treatment and remission…
Health and Demographic Characteristics of Patients Attending a Newly-Opened Medical Facility in a Remote Amazonian Community: A Descriptive Study
2018
Peru is a country with wide regional disparities in health. Remote Amazonian communities have high rates of poverty and poor access to health services. There is a lack of data on morbidity and use of health services in the region. We describe a descriptive, cross-sectional study of the demographic characteristics and presenting complaints of attendees to a newly-opened primary care facility in a remote community. This was supplemented by structured interviews of adult attendees to build a picture of sociocultural determinants of health locally, including engagement with traditional forms of medicine. Our study provides novel insights into an under-studied and under-resourced area. We found …
Das Pulmonalarteriensarkom - Prä- und postoperative radiologische Befunde bei der Tumorerstmanifestation und beim Rezidiv
2008
Purpose Primary pulmonary artery sarcomas are very rare tumors. Their diagnosis is difficult due to their unspecific symptoms. Still, an early preoperative diagnosis is the only possibility for a potential therapy. Patients and methods 4 pre- and 5 postoperative CT- and three pre- and one postoperative MRT-examination(s) have been evaluated with respect to surgical and histological findings. Results The presence of a convex intraluminal bulging mass spreading mostly from the pulmonary trunc continuously into the peripheral pulmonary artery branches the filling and dilatation of the vessels, the tumor's inhomogeneity indicating hemorrhages and necrosis and the appearance of intrapulmonary no…
Multicenter and all-comers validation of a score to select patients for manual thrombectomy, the DDTA score.
2021
Background Routine manual thrombectomy (MT) is not recommended in primary percutaneous coronary intervention (P-PCI) but it is performed in many procedures. The objective of our study was validating the DDTA score, designed for selecting patients who benefit most from MT. Methods Observational and multicenter study of all consecutive patients undergoing P-PCI in five institutions. Results were compared with the design cohort and the performance of the DDTA was analyzed in all patients. Primary end-point of the analyses was TIMI 3 after MT; secondary endpoints were final TIMI 3, no-reflow incidence, in-hospital mortality and in-hospital major cardiovascular events (MACE). In-hospital prognos…
