A case report of pulmonary vein isolation performed in a patient with polysplenia and interrupted inferior vena cava

2021

Abstract Background Pulmonary vein isolation (PVI) has entrenched itself as one of the main approaches for the treatment of paroxysmal symptomatic atrial fibrillation (AF). Pulmonary vein isolation prevents focal triggers from pulmonary veins from initiating AF paroxysms. As standard—PVI is performed through the inferior vena cava (IVC) approach, through the femoral vein. However, there are conditions when this approach is not appropriate or is not available. Case summary We report a case of a 53-year-old male who was referred to Pauls Stradins Clinical University Hospital for PVI due to worsening AF. Due to the rare anatomical variant of the venous system, the standard approach to PVI coul…

Development of a modular room-temperature hydride storage system for vehicular applications

2016

The subject of this paper concerns the development of a vehicular hydrogen tank system, using a commercial interstitial metal hydride as storage material. The design of the tank was intended to feed a fuel cell in a light prototype vehicle, and the chosen hydride material, Hydralloy C5 by GfE, was expected to be able to absorb and desorb hydrogen in a range of pressure suitable for this purpose. A systematic analysis of the material in laboratory scale allows an extrapolation of the thermodynamic and reaction kinetics data. The following development of the modular tank was done according to the requirements of the prototype vehicle propulsion system and led to promising intermediate results…

Measure the Embodied Energy in Building Materials: An Eco-Sustainable Approach for Construction

2019

This paper highlights how the use of materials and building components often implicates the growth of embodied energy necessary to their construction, which is not always adequately compensated by a decrease of operational energy because incorporated energy can be almost half of the total energy used in a building’s life cycle and, sometimes, it even exceeds operational energy. The paper highlights how searching only for “operational” energy efficiency does not sufficiently guarantee environmental sustainability of the intervention. The intervention is heavily influenced by embodied energy whose knowledge must drive, since the beginning, the decision-making process towards more sustainable …

Argentum-quarz solution in the treatment of anorectal fistulas: Is it possible a conservative approach?

2012

Patients suffering from chronic intestinal diseases (Crohn's disease, Ulcerative Colitis, Indeterminate Colitis) are prone to the development of pyogenic complications. These complications are most commonly in the form of perianal or intraabdominal abscesses and/or fistulas. The treatment of these complications are managed differently but, after an initial treatment based on medical or minimally invasive management, the solution of the pathological condition is always achieved by a surgical procedure. In the last few years prospective studies have proposed an alternative conservative therapeutic approach based on application of fibrin glue in the healing of patients with fistulas-in-ano. In…

Current Concepts and Perspectives on Brain Arteriovenous Malformations: A Review of Pathogenesis and Multidisciplinary Treatment.

2021

Brain arteriovenous malformations (bAVMs) are unusual vascular pathologies characterized by the abnormal aggregation of dilated arteries and veins in the brain parenchyma and for which the absence of a normal vascular structure and capillary bed leads to direct connections between arteries and veins. Although bAVMs have long been believed to be congenital anomalies that develop during the prenatal period, current studies show that inflammation is associated with AVM genesis, growth, and rupture. Interventional treatment options include microsurgery, stereotactic radiosurgery, and endovascular embolization, and management often comprises a multidisciplinary combination of these modalities. T…

Path Dependence and Paradox in Harmonizing Out-of-court Procedures across Europe. The Evidence from Italy

2018

This paper focuses on the impact that the ‘new approach to business failure’ has had on Italian out-of-court procedures. It will demonstrate that in 2005 Italian law started to embrace the rescue culture of out-of-court procedures by means of a series of reforms; initially, this movement facilitated the incorporation of the ‘new approach to business failure’, but – and this is the paradox – the more law makers and courts remove the old paradigms and introduce new ones, which in principle could make the procedures smoother, cheaper and more efficient, the more the law in the book and the law in action appear to be overloaded with additional prerequisites which make the procedures cumbersome…

Iterative closure method for non-linear systems driven by polynomials of Gaussian filtered processes

2008

This paper concerns the statistical characterization of the non-Gaussian response of non-linear systems excited by polynomial forms of filtered Gaussian processes. The non-Gaussianity requires the computation of moments of any order. The problem is solved profiting from both the stochastic equivalent linearization (EL), and the moment equation approach of Ito's stochastic differential calculus through a procedure divided into two parts. The first step requires the linearization of the system, while retaining the non-linear excitation; the response statistical moments are calculated exactly, and constitute a first estimate of the moments of the actual non-linear system. In the second step, t…
