Silicone-Silver Penile Prosthesis: Description, Operative Approach and Results

A silicone penile prosthesis in which silver wires are embedded to allow for voluntary bending of the penis for urination and resting position and for straightening for intercourse is described. The implantation is done via a hemicircular incision on the dorsum of the penis in the sulcus coronarius. Buck's fascia is identified and opened, and the corpora are dilated up to 30F. After the exact length is determined with a special sizer the prosthesis is slipped into each corpus cavernosum. The advantage of this approach is the optimal insertion of the prosthesis distal to the sulcus coronarius, thereby preventing kinking of the glans during intercourse. Aims: An extensive colonization of surgical meshes with autologous fibroblasts may reduce complications. Therefore, we aimed to establish a technique that allows isolation and propagation of fibroblasts from vaginal biopsies. Using these cells we tested the applicability of several clinically applied meshes for fibroblast coating. Materials & methods: Fibroblasts were isolated from vaginal tissue after digestion with collagenase. Characterization was performed by immunostaining for cytokeratin 5, 6 and 14, smooth muscle actin and vimentin. The study was aimed to test the mechanical strength, structural stability, and tissue reactions of optically amorphous and crystalline polyetheretherketone (PEEK) plates during a 3-year follow-up in vivo and in vitro. The injection-moulded PEEK plates were implanted to the dorsal subcutis of 12 sheep, which were sacrificed at 6-156 weeks. Purpose: We evaluate the feasibility and potential advantages of ProACT system implantation using transrectal ultrasound rather than fluoroscopy for guidance. Materials and Methods: The transrectal ultrasound guided procedure was done between June and October 2005 in 7 patients with a mean age of 68.4 years (range 53 to 76) with mild to severe stress urinary incontinence after laparoscopic transperitoneal radical prostatectomy.
