This is the second of two papers concerning our study into an integrated approach to psychotic disorders conducted at the University Psychiatry Unit of Palermo's Polyclinic over approximately 15 years; this paper concentrates on the clinical phenomena. The study aimed to find the best possible treatment and to improve the prognosis of this patient group. We have explored the efficacy of a range of psycho-therapeutic (cognitive-behavioural, systemic-relational, psychodynamic, group and others), psycho-pharmaceutical, psychiatric rehabilitative and psycho-educational treatments, with a hermeneutic approach instead of a systematic one. The present article aims to provide an insight into the life story of a computer-game addicted adolescent. Here, the relationship between the symptom game addiction, the family as a reference framework, the game's characteristics, as well as the subjective emotional state of the adolescent are of particular interest. Children who live on the margins of society are disadvantaged in achieving their developmental potential because of the lack of a necessary stable environment and nurturing care. Many early prevention programs aim at mitigating such effects, but often the evaluation of their long-term effect is missing. The aim of the study presented here was to evaluate such long-term effects in two prevention programs for children-at-risk growing up in deprived social environments focusing on child attachment representation as the primary outcome as well as on self-reflective capacities of teachers taking care of these children. Zusammenfassung. In der Kinder- und Jugendlichenpsychiatrie und -psychotherapie findet die Operationalisierte Psychodynamische Diagnostik (OPD-KJ) in den letzten Jahren zunehmend Anwendung. Dieser Beitrag stellt die Konfliktachse der OPD-KJ vor, die eine Operationalisierung von sieben psychodynamischen Konflikten und den jeweils zugeordneten Verarbeitungsmodi enthält. Er beschreibt empirische Ergebnisse zum Vergleich der Konfliktachsenratings und des Strukturratings in einer ambulanten und einer stationären Gruppe von Kindern und Jugendlichen (gesamt N = 186, 12.7 Jahre, 54 % weiblich). Abstract Background Online self-help interventions have proven to be effective in treating various specific mental disorders, mainly depression and anxiety. Knowledge regarding their acceptance, efficacy, and usefulness in addition to inpatient or outpatient psychotherapy is limited. Influence of childhood adversities and defense styles on the 1-year follow-up of psychosomatic- psychotherapeutic inpatient treatment

Abstract The current study explores the connection between reported sexual abuse and physical maltreatment during childhood and immature defense styles as affecting the 1-year follow-up of inpatients who had undergone psychodynamic group therapy. Early traumatization was assessed using a structured patient interview and the defense styles by means of the questionnaire on defense styles. Reduction in psychological distress and improvement in health-related quality of life formed the parameters against which success of treatment was measured.
