Dynamic in vitro dehydration patterns of unworn and worn silicone hydrogel contact lenses.

Purpose: To evaluate the effect of wear on dynamic in vitro dehydration of silicone hydrogel (Si-Hi) contact lens (CL) using a previously described gravimetric procedure. Methods: Five different silicone hydrogel (Si-Hi) contact lenses (CL) were evaluated after being worn by patients under daily wear conditions for 15 days to 1 month. Lenses were conditioned and disinfected with a multipurpose solution after each day of wear. Lenses were left to dehydrate in an analytical balance under controlled conditions of temperature and relative humidity and the results compared with data from new lens samples of the same power and material. Spontaneous plaque rupture visualized by intravascular ultrasound.

An intravascular ultrasound examination was performed in order to evaluate an angiographically complicated lesion. Intravascular ultrasound was able to demonstrate spontaneous plaque rupture in a patient with recent acute subendocardial infarction. The inconclusive angiographic appearance was clarified by the intravascular examination and led us to conclude that the myocardial infarction was due to plaque rupture with subsequent thrombotic occlusion, which had spontaneously resolved by the time of the study.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) is defined by clinical evidence of myocardial infarction (MI) with normal or near-normal coronary arteries on angiography. This condition is present in about 5% to 25% of patients presenting with acute coronary syndromes. MINOCA is a working diagnosis. Introduction: It is not well established how the medication recommended in the guidelines will affect plaque compositional changes in non-culprit segments after STEMI. We hypothesized that the proportion of necrotic tissue in the non-culprit lesion in the culprit artery would decrease ten months after STEMI. Methods: In 63 consecutive STEMI patients the non-culprit segment in the culprit artery was analyzed with iMap intravascular ultrasound (IVUS) at the time of the index procedure and 10 months later. Clinical use of recombinant tumor necrosis factor-alpha is strongly limited by its severe toxicity, mainly cardiovascular, when systemically administered. Recent studies suggest that topical (intrapleural, intraperitoneal, intratumoral) administration is free of significant toxicity. Human recombinant tumor necrosis factor-alpha was administered intravesically, at a dose of 500 mg dissolved in 30 ml of phosphate buffer (pH7.6-7.8) plus 0.25% human albumin, weekly for two months to 18 patients with papillary transitional cell carcinoma of the bladder. Graves' orbitopathy (GO) is the main extrathyroidal manifestation of Graves' disease, though severe forms are rare. Management of GO is often suboptimal, largely because available treatments do not target pathogenic mech anisms of the disease. Treatment should rely on a thorough assessment of the activity and severity of GO and its impact on the patient's quality of life. Local measures (artificial tears, ointments and dark glasses) and control of risk factors for progression (smoking and thyroid dysfunction) are recommended for all patients.
