Modular megaprosthesis reconstruction for oncological and non-oncological resection of the elbow joint

2016

Abstract Background Reconstruction of large bone defects around the elbow joint is surgically demanding due to sparse soft tissue coverage, complex biomechanics and the close proximity to neurovascular structures. Modular megaprostheses are established reconstruction tools for the elbow, but only small case series have been reported in the literature. Methods Thirty-six patients who underwent reconstruction of the elbow joint with a modular megaprosthesis were reviewed retrospectively. In 31 patients (86.1%), elbow replacement was performed after resection of a bone tumour, whereas five non-oncological patients (13.9%) underwent surgery because of a previous failed elbow reconstruction. Fun…

CEMENTLESS ACETABULAR RECONSTRUCTION AND STRUCTURAL BONE-GRAFTING IN DYSPLASTIC HIPS

2006

BACKGROUND: Studies of acetabular reconstruction with use of cement and bulk bone graft have demonstrated increasing rates of cup failure in patients with dysplastic hips seven years after total hip arthroplasty. Comparable data on the long-term results of bulk bone-grafting done in conjunction with cementless implants are limited. The aim of this study was to review the clinical and radiographic results of autologous bulk bone-grafting in conjunction with a cementless cup. METHODS: From 1987 to 1992, forty-seven patients (forty women and seven men, with an average age of 50.4 years) who had developmental dysplasia of the hip underwent fifty-six total hip arthroplasties and received a struc…

Functional consequences of microbial shifts in the human gastrointestinal tract linked to antibiotic treatment and obesity

2013

The microbiomes in the gastrointestinal tract (GIT) of individuals receiving antibiotics and those in obese subjects undergo compositional shifts, the metabolic effects and linkages of which are not clearly understood. Herein, we set to gain insight into these effects, particularly with regard to carbohydrate metabolism, and to contribute to unravel the underlying mechanisms and consequences for health conditions. We measured the activity level of GIT carbohydrate-active enzymes toward 23 distinct sugars in adults patients (n = 2) receiving 14-d β-lactam therapy and in obese (n = 7) and lean (n = 5) adolescents. We observed that both 14 d antibiotic-treated and obese subjects showed higher …

Impact of Model Shape Mismatch on Reconstruction Quality in Electrical Impedance Tomography

2012

Electrical impedance tomography (EIT) is a low-cost, noninvasive and radiation free medical imaging modality for monitoring ventilation distribution in the lung. Although such information could be invaluable in preventing ventilator-induced lung injury in mechanically ventilated patients, clinical application of EIT is hindered by difficulties in interpreting the resulting images. One source of this difficulty is the frequent use of simple shapes which do not correspond to the anatomy to reconstruct EIT images. The mismatch between the true body shape and the one used for reconstruction is known to introduce errors, which to date have not been properly characterized. In the present study we…

The decision-making process for breast reconstruction after cancer surgery: Representations of heterosexual couples in long-standing relationships

2017

Objectives Most people deal with intrusive life events such as cancer and the care trajectory together with their intimate partners. To our knowledge, no research has studied the involvement of the partner in the decision-making process regarding breast reconstruction (BR) after cancer. This study aimed to gain a better understanding of the couples’ decision-making process for BR in the cancer context and particularly to investigate the partners’ involvement in this process. Method Eighteen participants (nine women who underwent a mastectomy following a first breast cancer and their intimate partners) took part in this study. We conducted semidirective interviews, and a general inductive ap…

Different Hydraulic Constructs to Optimize the Venous Drainage of DIEP Flaps in Breast Reconstruction: Decisional Algorithm and Review of the Literat…

2020

Venous congestion is the most common perfusion-related complication of deep inferior epigastric artery perforator (DIEP) flap. Several hydraulic constructs can be created for venous superdrainage in case of flap venous engorgement or as a preventive measure. These can be classified based on the choice of the draining vein of the flap, either a second deep inferior epigastric vein (DIEV) or a superficial inferior epigastric vein (SIEV), and of the recipient vein, either a vein of the chest or the DIEV. Background Venous congestion is the most common perfusion-related complication of deep inferior epigastric artery perforator (DIEP) flap. Several hydraulic constructs can be created for venou…

Reconstruction of mandibular defects using vascularized fibular osteomyocutaneous flap combined with nonvascularized fibular flap

2019

Background The height of single-layer fibular flap is not long enough to return to the ideal height of the mandible. While the double-layer vascularized fibular osteomyocutaneous flap(VFF) is more complicated in shaping and fixation, along with a longer operation time. The aim of this study was to investigate the clinical effect of VFF combined with nonvascularized fibular flap(NVFF) in the reconstruction of mandibular defect. Material and Methods From September 2016 to June 2018, 15 patients with benign mandibular tumors underwent reconstruction with VFF and NVFF. SimPlant Pro ™ software (version 11.04) was used to simulate reconstruction of the mandible preoperatively. Results All patient…

Insights Into the Inside - A Quantitative Histological Study of the Explosively Moving Style in Marantaceae.

2018

This study aims to identify the histological basis for the extraordinary, fast movement of the style in Marantaceae. Although this explosive pollination mechanism was subject of many studies, quantitative measurements to document volumetric changes have never been conducted. Based on physical parameters and limitations (poroelastic time), the movement itself is by far too fast to be explained by turgor changes solely. Therefore, we address the hypothesis that the style contains elastic structures to store energy allowing the fast movement. We provide an experimental approach in Goeppertia bachemiana to identify histological differences of styles in various states, i.e., steady, unreleased, …

CLINICAL OUTCOMES OF SELF-EXPAMDABLE METALLIC STENTS IN PALLIATION OF MALIGNANT ANASTOMOTIC STRICTURES: A SINGLE CENTER EXPERIENCE

2015

Background: self-expandable metallic stents (SEMS) are employed as the preferred non surgical palliative treatment for gastric outlet obstruction due to malignancies. Metallic stents are often employed to treat malignant anastomotic obstructions after surgicsl interventions as esophagojejunostomy, gastrojejunostomy and esophagogastrojejunostomy. Methods: this case series reports prospectively the clinical outcomes of SEMS in the palliative care of malignant anastomotic strictures caused by the recurrence gastric cancer follwing gastric surgery as oncological curative treatment, in a series of nine consecutive patients, treated between January 2009 and december 2012 in our center. Results: N…
