Pulsatile versus continuous oxytocin infusion for the oxytocin challenge test.

In a prospective study, 140 patients had an oxytocin challenge test with either a continuous or a pulsed infusion (one minute of infusion in every five minutes). Both infusion regimens had similar success rates in terms of uterine contractions (97.1 vs 98.6%). The potency ratio (pulsed versus continuous infusion) was significant at 2.7 (1.27 to 5.2), which means that more uterine activity was induced with each mU of oxytocin with pulsatile than with continuous administration. A 20-month-old girl showed typical clinical signs of Farber disease: hoarseness since birth, and periarticular subcutaneous painful nodules. Complete deficiency of acid ceramidase activity was found in cultured skin fibroblasts. An electron microscopic examination of a dermal nodule disclosed pathognomonic tubular inclusions in histiocytes. In epidermal cells zebra-body-like and needle-like lysosomal inclusions were found. Summary As of May, 2002, 128 cases of variant Creutzfeldt-Jakob (vCJD) disease have been identified in the UK, France, and Ireland. We report the first case of vCJD in Italy. The patient was a young Italian woman who had never travelled to a country with bovine spongiform encephalopathy (BSE). She was diagnosed by cerebral MRI and western blot analysis of tonsil biopsy samples. The results of these analyses suggest that vCJD in continental Europe and the UK share genetic, clinical, and neuroimaging features, with similar western blot patterns.
