A 70-year-old woman with a history of radical cystectomy and continent urinary diversion with a Mainz pouch I due to muscle invasive bladder carcinoma presented for a routine evaluation. Physical examination, electrocardiogram, chest x-ray and routine laboratory values showed no pathological findings. There was no history of salivary gland disease. Excretory urography was performed with 100 ml. nonionic contrast medium (iopromide) injected intravenously. Introduction In the rare case of a decompensated neurogenic bladder combined with a unilateral hydronephrosis and hydroureter, ureterocystoplasty is a feasible and elegant technique for bladder augmentation. This manuscript will detail our experience using a microscopic endonasal surgery (MES) approach in the repair of spontaneous, traumatic, and/or iatrogenic cerebrospinal fluid (CSF) fistula into the ethmoid and sphenoid sinuses. The location of the fistula in 22 patients was determined by a direct endoscopic or microscopic visualization, intrathecal fluorescein, or computed tomography (CT) with metrizamide. Several of these patients had been treated unsuccessfully with previous neurosurgical, intracranial, and extracranial approaches to stop the leak. One of the major obstacles in organ transplantation is to establish immune tolerance of allografts. Although immunosuppressive drugs can prevent graft rejection to a certain degree, their efficacies are limited, transient, and associated with severe side effects. Induction of thymic central tolerance to allografts remains challenging, largely because of the difficulty of maintaining donor thymic epithelial cells in vitro to allow successful bioengineering.
