Cutaneous Adverse Reactions to Highly Antiretroviral Therapy in HIV-Positive Patients

Adverse drug reactions to highly antiretroviral therapy (HAART) are major obstacles in its success. Although overall mortality from HIV has dramatically declined owing to HAART, these antiretroviral regimens have been associated with a wide spectrum of severe cutaneous reactions. The severity of cutaneous adverse reactions varies greatly, and some may be difficult to manage. To optimize adherence and efficacy of antiretroviral treatment, clinicians must focus on preventing adverse effects whenever possible, and distinguish those that are self-limited from those that are potentially serious. Different face skin diseases (basal cell carcinoma, actinic keratosis, rosacea, solar elastosis, etc.) could clinically manifest itself as erythematic patches, pimples or plagues. It is very hard to make the clinical exclusion in some cases of these diseases since their characters can partially overlap or certain lesion can mimic another one especially in the cases of skin areas affected with sun. Therefore, the histopathological analysis remains the “golden standard” of the dermatological diagnosis at skin diseases. Ustekinumab therapy changes the transcriptional activity pattern of TGF-β1–3 genes

Introduction One of the examples of genes whose expression can be altered by the action of ustekinumab is TGF-β. It is a pleiotropic cytokine whose activity affects psoriatic changes and the state of homeostasis of the whole organism. Aim To evaluate the effect of ustekinumab on the transcriptional activity of TGF-b family genes in patients with psoriatic arthritis and to check whether the results obtained can be helpful in monitoring the progress of treatment. Milker’s nodules, also called paravaccinia, is a DNA virus transmitted from infected cows to humans. It results from contact with cattle, cattle byproducts, or fomites. Classified as an occupational disorder, those at risk of exposure include farmers, butchers, and agricultural tourists. The viral infection begins 5—15 days after inoculation as an erythematous-purple, round nodule with a clear depressed center, and a surrounding erythematous ring. While familiar to those in farming communities, the presence of the nodule may be concerning to others, particularly the immunosuppressed.
