Bone marrow punctures and pain

Abstract We prospectively analysed pain in 263 patients induced by a frequent diagnostic procedure for oncologists, specifically the bone marrow puncture. Substantial pain (5 and more out of 10 on a numerical rating scale) was reported by 30.4% of patients, but physicians did not realize this procedure-related pain of patients in more than 50% of such punctures. The necessity for improved analgesia is emphasized by the fact, that at least 50% of patients experiencing substantial pain wished to receive concomitant medication in future punctures. In Finland the administrative responsibility for outpatient psychiatric care has in part been reorganized from specialized psychiatric treatment organizations to health centers. This change has been encouraged by economic pressures, but data about its impact on the quality of treatment and patient satisfaction are limited. Delirium is a neuropsychiatric syndrome often manifesting in acute disease conditions, and with a greater prevalence in the older generation. Delirium in the Emergency Department (ED) is a highly prevalent problem that typically goes unnoticed by healthcare providers. The onset of a delirium episode in the ED is associated with an increase in morbidity and mortality. Because delirium is a preventable syndrome, these statistics are unacceptable. Endoscope-assisted Craniotomy

We describe a surgical technique to improve visualization of deep brain structures during craniotomy. The technique, developed in a cadaveric model, combines the use of an operating microscope and a solid-rod lens endoscope. Addition of the endoscope during craniotomy allows the operator to visualize structures that otherwise might not have been seen. An approach to the brain stem is demonstrated.

BACKGROUND We have made a comparison of clinical evaluation, Heidelberg-Retina-Tomograph (HRT) and Nerve Fiber Analyzer (GDx) in diagnosing glaucoma. MATERIALS AND METHODS One eye of 38 glaucoma patients and 26 non-glaucoma patients was examined with HRT and GDx. Assignment of the patients into the two groups by clinical evaluation was performed on the basis of visual field and optic disc results. As classification criterion for glaucoma/non-glaucoma by means of the instruments we employed the statistical classification of the HRT and "the number" of the GDx. Internet Addiction: Starting the Debate on Health and Well-Being of Children Overexposed to Digital Media.

Previous articles published in the European Paediatric Association (EPA) pages have discussed the importance of children's well-being and its role in different areas of children's health, such as well-child care and child-friendly health care. The goal of this commentary is to raise pediatricians' awareness on the risks of children's overexposure to digital media. It is important to protect the psychological and emotional health of children from uncontrolled external influences to preserve their well-being. We also emphasize the importance of alerting parents and tutors to the risks related to the unrefrained use of the Internet.

Adrenal gunshot wound: Laparoscopic approach. Report of a case

introduction: Although there is no debate that patients with peritonitis or hemodynamic instability should undergo urgent laparotomy after penetrating abdominal injury, it is also clear that certain stable patients may be managed without operation. Controversy persists regarding use of laparoscopy. PRESENTATION OF CASE: We report a case of gunshot wounds with bullet in left adrenal gland and perirenal subcapsular hematoma. The patients had no signs of peritonitis but in the observation period we noted a significative blood loss, so we performed an exploratory laparoscopy.
