Therapeutic properties of haemodialysis and blood exchange transfusion in organophosphate poisoning

Human blood was contaminated with nitrostigmine, dimethoate and demeton-S-methyl sulfoxide. It was then dialysed, concentrations of organophosphates were determined and dialysance values calculated. The influence of blood exchange transfusion on poison elimination as well as on the cholinesterase activity of blood, brain and muscle was studied in rats poisoned with nitrostigmine. Haemodialysis was found to be quite an effective method for eliminating demeton-S-methyl sulfoxide and dimethoate, dialysance values of 52.98 ml/min and 59.07 ml/min being found for demeton-S-methyl sulfoxide and dimethoate respectively. A case of a haemodialysis patient with a primitive angiosarcoma of the alveolar mucosa is reported. The vascular origin of the tumor was confirmed by the immunohistochemical data which showed strong positivity for Factor VIII-related antigen and for vimentin, whereas stains for desmin and cytokeratins were negative.

Hyperuricemia (HU) and tumour lysis syndrome (TLS) are complications of acute leukaemia and non-Hodgkin lymphoma (NHL) leading to increased morbidity and mortality. The objective of this study was to define incidence and calculate health care cost associated with HU and TLS. 788 acute leukaemia and NHL patients from Belgium, The Netherlands, Spain and UK were screened retrospectively for HU and TLS. Resource use related to HU and TLS was recorded and costs were calculated applying local unit costs. Results showed that HU occurred in 18.9% of patients, and 27.8% of them fulfilled TLS criteria. Transthoracic electrical impedance during extracorporeal hemodialysis in acute respiratory failure ("Shocked Lung syndrome").

The alteration (delta Z0) of transthoracic electrical impedance (TEI) during extracorporeal hemodialysis (EHD) was investigated in two Groups of patients with acute renal and acute respiratory failure, that differed with respect to the severity of respiratory insufficiency. Group I had moderate respiratory failure (FiO2 0.31 +/- 0.10, PaO2 84 +/- 14 mmHg), and Group II had severe respiratory failure (FiO2 0.75 +/- 0.17, PaO2 77 +/- 14 mmHg).
