Unilateral adrenal hyperplasia is a rare cause of primary hyperaldosteronism (around a 3%) that has surgical treatment. A case of a patient with hypertension resistant to conventional therapy in treatment with 7 drugs who presented with primary hyperaldosteronism due to unilateral adrenal hyperplasia is presented. A left adrenalectomy was performed, and the patient had a good clinical response, with no need of any drug after 2 years of surgery. Unilateral adrenal hyperplasia is a different entity and it is not an asymmetric variant of the bilateral adrenal hyperplasia. Background: From March 1994 to August 1995 we performed extraperitoneal endoscopic adrenalectomy in 18 patients with adrenal gland tumors. Methods: Two of these patients underwent bilateral adrenalectomy. For the extraperitoneal approach a pneumoret-roperitoneum was established and three 10-mm trocars were inserted in the area of the conventional flank incision. Laparoscopy versus open adrenalectomy in patients with solid tumor metastases: results of a multicenter European study

Background: The outcome of adrenalectomy carried out by laparoscopy or open surgery for solid tumor metastases was assessed. Methods: A total of 317 patients with histologically confirmed adrenal metastatic disease collected from 30 centres in Europe underwent adrenalectomy by laparoscopy (n=146) or open laparotomy (n=171). Differences between laparoscopic and open adrenalectomy were assessed by a single Cox analysis for both procedures. Results: The median overall survival was 24.0 [95% confidence interval (CI): 21.4-26.6] months for open adrenalectomy and 45.0 (95% CI: 22.6-67.4) for laparoscopic adrenalectomy (P=0.008). Vari studi hanno dimostrato come la stiffness arteriosa aumenti con il progressivo deterioramento della funzione renale. Una correlazione inversa è stata altresì dimostrata fra livelli plasmatici di endotelina-1 (ET-1) e filtrato glomerulare (GFR). Dati sperimentali hanno evidenziato che l’infusione di ET-1 è seguita da un incremento della stiffness arteriosa ed il trattamento con antagonisti del recettore ET-A dell’ET-1 in pazienti nefropatici ha comportato una riduzione della rigidità aortica, in parte indipendentemente dagli effetti sulla pressione arteriosa.
