Orthopedic and neurosurgical treatment of severe kyphosis in myelomeningocele

1999

Kyphosis in myelomeningocele is characterized by a complex pattern of problems during development and therapy. On the one hand, decompensation of upright posture leads to loss of sitting ability and social integration; on the other hand, accompanying malformations and trophic alterations threaten the physical integrity and performance. Neurologic function, cerebrospinal fluid (CSF) circulation, skeletal deformity and the urinary transport system need to be kept in mind and need to be treated with cooperation between the different specialties. Especially during serious surgical interventions such as spinal surgery, neither the nervous system nor the kidneys must be ignored. Sixteen patients …

Lymph Node Metastases in Non-Muscle Invasive Bladder Cancer are Correlated With the Number of Transurethral Resections and Tumour Upstaging at Radica…

2005

The first paper in this section, from Mainz, attempts to identify the clinical variables associated with the prevalence of lymph node metastases in non-muscle invasive bladder cancer. The authors found that delay in cystectomy in this potentially dangerous type of tumour is to be avoided, with a higher incidence of lymph node metastases as the number of transurethral resections increases. A paper from Austria shows that in renal carcinoma the pT1 subdivision is associated with differences in conventional histopathology and expression of biomarkers. OBJECTIVE To identify clinical variables associated with the prevalence of lymph node metastases (LNMs) in patients with non-muscle invasive tra…

Use of ADCON®-L to Prevent Peridural Fibrosis Following Re-Operation for Recurrent Lumbar Radiculopathy: Clinical Results

2002

OBJECTIVE To present the results of a small retrospective study in patients after they have undergone lumbar scar resection and ADCON-L application to prevent recurrent formation of peridural fibrosis. PATIENTS AND METHODS Between May 1996 and December 1999 nineteen patients underwent surgery for peridural fibrosis. Sixteen patients were eligible for statistical analysis. The mean age was 46.2 years (range 29 to 69 years) and the mean follow-up period was 9.7 months with a range of 3 to 38 months. In 10 patients scar formation was the main factor for nerve root compression. Three out of these patients showed concomitant recurrent disc herniation. Six patients presented with peridural fibros…

Long-Term Results After Liver Transplantation With “Livers That Nobody Wants” Within Eurotransplant: A Center's Experience

2008

Abstract Background Orthotopic liver transplantation (OLT) represents the only curative treatment for end-stage liver disease, but its application is limited because of organ shortages. The purpose of this study was to review the long-term outcomes after OLT during a 2-year period of 45 rescue offers organs within Eurotransplant. Patients and Methods Forty-five deceased donor liver allografts had been officially offered to and rejected by other transplantation centers 162 times prior to our acceptance. Data analysis addressed recurrence of primary disease, ischemic-type biliary lesions (ITBL), re-evaluation or relisting for OLT, re-OLT, as well as overall patient and graft survivals. Result…

The role of CARD15 mutations and smoking in the course of Crohn's disease in a Mediterranean area.

2008

To evaluate the role of CARD15 mutations and smoking in the main events of Crohn's disease (CD).A total of 182 patients with CD were included in a prospective study in order to evaluate the role of CARD15 mutations and smoking in the main outcomes of disease course: first operation and surgical recurrence. The following variables were evaluated in a univariable and multivariable analysis: age, sex, site of disease, pattern, smoking habit, extraintestinal manifestations, duration of disease, and CARD15 mutation. The Kaplan-Meier method for survival curves and Cox model for multivariable analysis were, respectively, used.A total of 110 patients were operated on and 32 were reoperated on. The …

High frequency rotational ablation: an alternative in treating coronary artery stenoses and occlusions.

1993

OBJECTIVE--To prove the safety and effectiveness of high frequency rotational ablation of coronary artery stenoses and occlusion in humans. SUBJECTS--106 patients with symptoms (91 men, 15 women) who had 67 significant stenoses, mainly types B and C, and 46-chronic occlusions. MAIN OUTCOME MEASURES--Mean change in diameter stenosis after rotational angioplasty alone and in combination with percutaneous transluminal coronary angioplasty immediately after treatment and 24 hours and six months later; restenosis rates at six months; complication of treatment. RESULTS--Rotational ablation could not be used in five stenoses and 16 chronic occlusions because of inability to reach or cross the lesi…

Endothelial study of iris-claw phakic lens: four year follow-up.

1998

Abstract Purpose: To study quantitative and morphometric endothelial changes in phakic eyes implanted with the Worst iris-claw lens to correct high myopia. Setting: Department of Ophthalmology, University Hospital “La Fe”, Valencia, Spain. Material and Methods: This retrospective study involved 111 phakic eyes (73 patients) implanted with the Worst iris-claw lens. Noncontact specular microscopy and computer-assisted analysis was performed preoperatively and 6 months and 1, 2, 3, and 4 years postoperatively. Results: The mean cell loss was 3.85% at 6 months, 6.59% at 1 year, 9.22% at 2 years, 11.68% at 3 years, and 13.42% at 4 years. At 2 years, the hexagonality and coefficient variation in …

Revisional bariatric surgery due to failure of the initial technique: 25 years of experience in a specialized Unit of Obesity Surgery in Spain.

2019

Abstract Objectives To evaluate the effectiveness of conversion surgery in a bariatric surgery unit with 25 years of experience. Method Retrospective observational study of patients with type II obesity or higher who were reoperated by means of conversion surgery due to weight regain, residual body mass index (BMI) >35 kg/m2 or Results A total of 112 patients were included, with a mean age of 40.2 years, who initially underwent vertical banded gastroplasty (VBG) (32.1%), gastric banding (GB) (23.2%), Roux-en-Y gastric bypass (RYGB) (21.4%) and sleeve gastrectomy (SG) (23.2%). The conversion techniques, with a median time between the two surgeries of 70 months, included: RYGB, SG, one-anasto…

Management of Cochlear Implant Electrode Migration.

2016

The present study reviewed a cochlear implant (CI) patient population after surgery, which received a free-fitting electrode carrier designed for hearing preservation. The aim was to determine the rate of electrode migration of the CI electrodes and present clinical and surgical implications.Retrospective patient review.Tertiary referral university hospital. Two hundred seventy-eight patients implanted uni- or bilaterally with lateral wall electrodes designed for hearing preservation (358 implants). The control group was 323 patients implanted uni- or bilaterally with preformed perimodiolar electrodes (468 implants).Determination of CI electrode migration was conducted according to a clinica…

Questionable Value of Adjuvant Arteriovenous Fistula in Pedal Bypass at High Risk for Early Failure

2007

Results of an adjuvant arteriovenous fistula (AVF) in pedal bypass surgery in the presence of poor status of the recipient artery, severely impaired intraoperative runoff, or revision for early failure and flow restitution were analyzed in a retrospective study. From January 1998 to December 2006, 24 adjuvant AVFs were constructed in autologous vein or composite pedal bypasses with low intraoperative bypass flow, poor status of the pedal artery, or during successful early bypass revision to prevent graft failure. All infrainguinal bypass operations were registered in a computerized database and prospectively followed. Pedal bypasses with adjunctive AVF were reviewed for fistula function, gr…
