Randomised clinical trials: a source of ethical dilemmas

Clinical trials give rise to ethical dilemmas, especially in the acutely ill, but we take issue with two points raised in a recent comment on a specific acute myocardial infarction (AMI) trial. The commentators judged that the trial most likely could, and therefore should, have been terminated much earlier. By analysing the problem statistically we arrive at results that go against their intuitive judgment—they also see it as mandatory to update the patient Information sheet as trial results accrue and trends begin to emerge. This article and the four videos linked to this article are a result of the earliest experiences in establishing an international research collaboration among seven countries in the Project Social and emotional experiences in transition through the early years. We draw attention to the complex issues surrounding the many processes, beliefs and attitudes about infants in research that permeated our processes of gaining ethical approval for the international study and which posed many challenges for our project. ResumenSe reporta una investigación cualitativa cuyo objetivo fue develar cómo infl la racionalidad instrumental de las políticas en las prácticas de los cuerpos académicos y la formación ética de estudiantes. Se reunieron 17 narrativas de profesores de dos universidades. El análisis combinó los procedimientos de teoría fundamentada y etnografía. Se encontró que: a) la racionalidad instrumental se refleja en las interacciones académicas y en las finalidades priorizadas; b) se busca formar en la ética del reconocimiento y el compromiso, pero mediante la socialización en la que afloran la reificación y la cooperación interesada.
