Biological Causes of Anterior Knee Pain

We review the pathophysiology of anterior knee pain in the young patient. Emphasis is placed on newer findings. We have developed what we call the “Neural Model” as an explanation for the genesis of anterior knee pain. We have demonstrated a neuroanatomical basis for PFPS in the young patient and the clinical observation that the lateral retinaculum may have a key role in the origin of this pain. Posterior Tibial Tendon Dislocation: A Case Report

Abstract Isolated posterior tibial tendon dislocation is a rare condition. Diagnosis can be challenging, especially in the acute clinical setting. Predisposing factors include shallow posterior tibial tendon groove and tear of the flexor retinaculum. We report the case of traumatic subluxation of the posterior tibial tendon, illustrating imaging findings and surgical technique. Posterior tibial tendon dislocation was detected using dynamic ultrasound, while magnetic resonance was essential for a comprehensive evaluation that included underlying predisposing anatomic abnormalities and associated pathologic conditions. Kinetic and Kinematic Analysis of Iatrogenic Medial Patellar Instability: Clinical Relevance

We speculate that chronic recurrent soft tissue fatigue or overload due to patellofemoral imbalance could cause anterior knee pain. However, many chondropathies can be asymptomatic due to the biomechanic defence strategies used by the patients. Moreover, we emphasize the importance of the passive restraining structures in patellar stability, in contrast to the role of muscle function advocated by some authors. Endoscopic carpal tunnel release: results with special consideration to possible complications.

Single-portal endoscopic carpal tunnel release (Agee technique) was carried out in 148 patients. Of these, 100 were examined clinically and electrophysiologically 1 day before surgery and 1 and 3 months thereafter. Ten patients experienced transient ulnar neuropraxia and two patients complained postoperatively of intense pain in the middle and ring fingers. Three patients suffered residual symptoms. In one of these an incomplete release of the distal portion of the retinaculum flexorum was suspected and later confirmed by magnetic resonance imaging.
