Malignant PEComa With Metastatic Disease at Diagnosis and Resistance to Several Chemotherapy Regimens and Targeted Therapy (m-TOR Inhibitor).
2017
Perivascular epithelioid cell tumors (PEComas) are infrequent neoplasms with peculiar myomelanocytic differentiation. The aggressive abdominopelvic variant is rare, with only a small number of published cases. We present an additional case of this unusual variant, which showed an aggressive histologic and clinical behavior with multiple liver metastases and resistance to several therapies. We also discuss the histological and immunohistochemical profiles as well as the differential diagnosis.
Imaging features of perineural and perivascular spread in rapidly progressive rhino-orbital-cerebral mucormycosis: A case report and brief review of …
2021
Background: Rhinocerebral mucormycosis (ROCM) is an opportunistic fungal infection originating from the paranasal sinuses with extension to the brain. A delayed diagnosis can rapidly result in a poor prognosis. ROCM commonly affects patients with diabetes or immunocompromised states with a variable progression. Case Description: We report the case of a 59-year old patient with an untreated diabetes who developed a ROCM with rapidly progressive neurological symptoms. From the onset of sinus pain, nasal congestion, he rapidly developed facial swelling and masticatory dysfunction. The patient underwent sinus surgery which allowed Rhizopus oryzae to be isolated. Accordingly, a systemic therapy…
High volume and low concentration of anaesthetic solution in the perivascular interscalene sheath determines quality of block and incidence of compli…
2006
Background and objective: In the perivascular sheath of the brachial plexus, the volume of anaesthetic solution determines the quality of anaesthetic cover. Fibrous septa may divide the perivascular space into compartments, leading to inadequate diffusion of the anaesthetic solution. The aim of our study was to obtain good anaesthesia and less complications using high volume of low concentration anaesthetic solution, overcoming the obstacle of the septa with a double approach to the scalene sheath. Methods: Sixty patients scheduled for shoulder capsuloplasty received both Winnie interscalene brachial plexus block and Pippa proximal cranial needle approach. The patients were randomly assigne…
Galantamine is an allosterically potentiating ligand of neuronal nicotinic but not of muscarinic acetylcholine receptors.
2003
Galantamine (Reminyl), an approved treatment for Alzheimer's disease (AD), is a potent allosteric potentiating ligand (APL) of human alpha 3 beta 4, alpha 4 beta 2, and alpha 6 beta 4 nicotinic receptors (nAChRs), and of the chicken/mouse chimeric alpha 7/5-hydroxytryptamine3 receptor, as was shown by whole-cell patch-clamp studies of human embryonic kidney-293 cells stably expressing a single nAChR subtype. Galantamine potentiates agonist responses of the four nAChR subtypes studied in the same window of concentrations (i.e., 0.1-1 microM), which correlates with the cerebrospinal fluid concentration of the drug at the recommended daily dosage of 16 to 24 mg. At concentrations10 microM, gal…
Fine-scale estimations of bioclimatic change in the Valencia region, Spain
2016
Recent advances in statistical downscaling have allowed the reconstruction of temperatures for the complete 1948–2011 period in a spatial resolution of 90 m and without gaps for the Valencian Community (Spain) and bordering areas. It presently enables analyses in this region, which allows the determination of recent temperature changes at subregional and local scales. The present work focuses on obtaining the thermicity index according to Rivas-Martínez, a well-known indicator of different thermotypes associated with bioclimatic horizons. The change in this index, which has happened in the region between 1948 and 2011, was calculated by generating fine-scale maps of the potential extension …

FLOW PATTERN AROUND THE RIGID CEPHALIC SHIELD OF THE DEVONIAN AGNATHANERRIVASPIS WAYNENSIS(PTERASPIDIFORMES: HETEROSTRACI)
2008
Palaeozoic armoured agnathans (or ostraco- derms) are characterised by having an external, bone shield enclosing the anterior part of their bodies, which demon- strate great diversity of both forms and sizes. The functional significance of these cephalic shields remains unclear (they may have been a functional analogue of the vertebral col- umn, or merely afforded protection). Here we assess the importance of the cephalic shield in terms of locomotion. In order to do this, we have studied flow patterns of the Devo- nian heterostracan Errivaspis waynensis (White, 1935), using an anatomically correct model of E. waynensis positioned at different pitching angles. The fluid flow was visualised …
On the tail ofErrivaspisand the condition of the caudal fin in heterostracans
2009
Articulated caudal fins of heterostracans are preserved in very rare cases. Their morphology and internal structure are completely unknown for the majority of species. One of the few preserved caudal fins belongs to the Early Devonian pteraspidid Errivaspis waynensis from the Welsh Borderland, UK. There are two different reconstructions of the tail: (1) strongly asymmetric, with a considerably longer ventral lobe, and a rather concave posterior margin (e.g. White 1935); and (2) almost symmetrical with an only slightly longer ventral lobe and a small notch in the posterior margin (e.g. Blieck 1984). A new reconstruction of the Errivaspis caudal fin, based on a detailed study of White's speci…
Pharmacokinetic rationale for switching from donepezil to galantamine.
2001
Galantamine, the most recently approved acetylcholinesterase inhibitor (AChEI) for use in the United States, has allosteric modulating activity at nicotinic receptors and inhibits acetylcholinesterase. This dual mechanism of action may make galantamine an attractive option for patients with Alzheimer's disease who have not benefited from their current therapy; thus, methods for switching patients from donepezil or rivastigmine to galantamine are needed. Protocols for switching patients from one AChEI to another must consider both the time required for washout of the first drug and the rate of dose escalation of the second drug. Both issues depend on the pharmacodynamics, pharmacokinetics, a…
The pia mater at the site of the entry of blood vessels into the central nervous system
1982
The entry of blood vessels into the central nervous system (CNS) has been studied at the surface of the brain stem and the spinal cord of two cats and two rabbits. The study was carried out by scanning electron microscopy (SEM). The results obtained by SEM concerning the layers of the pia mater corresponded to those obtained by other authors who used transmission electron microscopy. Between the basal lamina of the superficial glial membrane of the CNS and the cells lining the subarachnoid space, a pial connective tissue space could be clearly recognized. The blood vessles crossing the subarachnoid space were also covered by leptomeningeal lining cells. At the site of entry of these blood v…
