Evaluation of oral anticoagulation therapy: Rationale and design of the thrombEVAL study programme

2013

Since decades, oral anticoagulation (OAC) with vitamin K antagonists (VKA) is an established therapy for both prevention and treatment of thromboembolism in daily clinical routine. Increasing life expectancy, demographic changes, and novel oral anticoagulants have led to an increasing complexity of medical therapy. However, data on quality and management of VKA therapy with phenprocoumon in current medical care are limited. Our aim is to investigate the quality of OAC with VKA in current health care and to evaluate the potential for improvements. The investigator-initiated thrombEVAL study programme comprises two cohorts of patients treated with vitamin K antagonists for oral anticoagulation…

Metastatic Versus Osteoporotic Vertebral Fractures on MRI: A Blinded, Multicenter, and Multispecialty Observer Agreement Evaluation.

2020

Background: MRI is assumed to be valid for distinguishing metastatic vertebral fractures (MVFs) from osteoporotic vertebral fractures (OVFs). This study assessed (1) concordance between the image-based diagnosis of MVF versus OVF and the reference (biopsy or follow-up of >6 months), (2) interobserver and intraobserver agreement on key imaging findings and the diagnosis of MVF versus OVF, and (3) whether disclosing a patient’s history of cancer leads to variations in diagnosis, concordance, or agreement. Patients and Methods: This retrospective cohort study included clinical data and imaging from 203 patients with confirmed MVF or OVF provided to 25 clinicians (neurosurgeons, radiologists…

OP0089 METABOLIC SYNDROME IS COMMON AROUND THE TIME OF EARLY RHEUMATOID ARTHRITIS DIAGNOSIS AND ASSOCIATIONS VARY BY SEX AND MENOPAUSAL STATUS: RESUL…

2019

Background Prevalence studies of metabolic syndrome (MetS) in early rheumatoid arthritis (ERA) are sparse and estimates are variable. Differences by sex and menopausal status have not been formally assessed. Objectives We estimated prevalence and factors associated with MetS in ERA patients treated in routine practice settings. Variations by sex, and by menopausal status were also examined. Methods Cross-sectional data were analyzed from patients meeting 1987 or 2010 ACR/EULAR RA criteria enrolled in the Canadian Early Arthritis Cohort (CATCH) from January 2007-March 2017. CATCH is a prospective multicenter observational study of patients diagnosed and treated for early inflammatory arthrit…

The use of contrast echocardiography: a matter of clinical judgement

2005

In their editorial, Buck and Erbel1 raise the important question in which settings application of contrast agents to an echocardiographic examination is appropriate. There is no doubt that the beauty of echocardiography relates to its ease of use. We should acknowledge that highest accuracy in the determination of left ventricular function is not necessary in most patients referred for an echocardiographic evaluation. Thus, contrast echocardiography will not be a technique to be used in all patients for mere evaluation of left ventricular function as cardiac magnetic resonance imaging will not be used in routine clinical practice only for its known superb accuracy. However, there are patien…

Advanced Endoscopic Imaging Methods

2021

Endoscopic imaging is the mainstay of gastrointestinal endoscopy and has experienced remarkable improvements in recent decades. Further endoscopic innovations stemming from major technological advances such as artificial intelligence and deep learning are at advanced stages of development and will be increasingly deployed in routine clinical practice. These will improve the detection and enable real-time optical diagnosis of pathologies in the gastrointestinal tract. Furthermore, the utilization of endoscopic robots may result in higher diagnostic yield and greater patient comfort. This chapter covers the most recent developments around computer-assisted diagnosis, hypoxia and three-dimensi…

Reliability and validity of time domain heart rate variability during daily rout¡ne activities : an alternative to the morning orthostatic test?

2017

Summary Study aim: To determine the reliability and validity of a time domain heart rate variability (HRV) index during free-living physical activity (FLPA). Material and methods: Eight white-collar workers participated in this study. RR intervals (time between consecutive R-peaks of the PQRS complex) were recorded using the Polar V800 heart rate (HR) monitor upon awakening and at work on 16 different days. A total of 127 cycles of sitting periods followed by walking breaks were included for consecutive pairwise analysis for reliability. The HR values from the orthostatic test (OT) were compared with the corresponding values at work. Results: The HR values showed high levels of repeatabilit…

Pharmacokinetics of new oral anticoagulants: implications for use in routine care

2018

Introduction: Since 2008, new oral anticoagulants (NOACs) have been approved for the prevention of venous thromboembolism (VTE) in patients receiving hip or knee replacement surgery, prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation (NVAF), treatment of deep vein thrombosis (DVT), and pulmonary embolism (PE). Premarketing randomized clinical trials (RCTs) of NOACs demonstrated their non-inferiority in terms of efficacy vs. warfarin (traditional oral anticoagulant–TOA), with lower risk of serious adverse drug reactions, especially cerebral hemorrhages. In clinical practice, pharmacokinetic aspects of NOACs have to be carefully taken into account to …

Towards a routine-based view of interfirm rivalry

2022

Although organizational routines have attracted increasing attention in strategy and organization research, they have received surprisingly limited attention in competitive dynamics scholarship. Our essay seeks to advance a routine-based view of interfirm rivalry by bridging the competitive dynamics and routine literatures. We put forward a conceptual model of the routine-based view of interfirm rivalry that is centered on “competitive action routines.” The model clarifies the roles that managers play in driving a firm’s competitive behavior, challenges the assumption of routine-based rigidity in competitive behavior, and adds nuance to our understanding of managerial cognition in competiti…

Hacia la implementación de un modelo de atención temprana por parte de una Organización Paraguaya

2020

Implementation science has been developed to help understand why evidence-based practices are not adopted as rapidly as they might. We describe here the process a rehabilitation center in Paraguay is undergoing to transform itself into a state-of-the-art early intervention program. We describe the organization and its contextual strengths and barriers, the model it chose to implement, and the implementation procedures it has attempted. The implications for model fidelity are highlighted, as this organization needed to make adaptations to the model as designed, to fit the particular context of its mission and the Paraguayan culture. Features of the Routines-Based Model are described.  La cie…
