The effect of RAAS blockade on markers of renal tubular damage in diabetic nephropathy: u-NGAL, u-KIM1 and u-LFABP.

2012

Blockade of the renin-angiotensin-aldosterone system (RAAS) affects both the glomerulus and tubules. We aimed to investigate the effect of irbesartan on the tubular markers: urinary (u) neutrophil gelatinase associated protein (NGAL), Kidney injury molecule 1 (KIM1) and liver-fatty acid-binding protein (LFABP).A substudy of a double-masked, randomized, cross-over study including 52 patients with type 2 diabetes, hypertension and microalbuminuria. After 2 months washout of all antihypertensive medication except bendroflumethiazid, patients were treated in random order with irbesartan 300, 600 and 900 mg for 2 months.Urinary tubular markers at baseline and after each treatment period (ELISA),…

Effect of a Strategy of Comprehensive Vasodilation vs Usual Care on Mortality and Heart Failure Rehospitalization Among Patients With Acute Heart Fai…

2019

Importance Short-term infusions of single vasodilators, usually given in a fixed dose, have not improved outcomes in patients with acute heart failure (AHF). Objective To evaluate the effect of a strategy that emphasized early intensive and sustained vasodilation using individualized up-titrated doses of established vasodilators in patients with AHF. Design, Setting, and Participants Randomized, open-label blinded-end-point trial enrolling 788 patients hospitalized for AHF with dyspnea, increased plasma concentrations of natriuretic peptides, systolic blood pressure of at least 100 mm Hg, and plan for treatment in a general ward in 10 tertiary and secondary hospitals in Switzerland, Bulgari…

Efficacy in angiotensin receptor blockade: a comparative review of data with olmesartan

2009

A range of angiotensin II receptor blockers (ARB) is available, and analyses suggest there are differences between agents in terms of antihypertensive efficacy and 24-hour blood pressure control. This review assesses the data comparing olmesartan with other ARBs in terms of blood pressure reductions, goal achievement, 24-hour control and speed of onset. Olmesartan seems to have a more favourable efficacy profile relative to standard doses of the ARBs used in comparative studies; results consistent with the high degree of blockade of the angiotensin II type 1 receptor for olmesartan.Taken together, there might be differences between ARBs regarding their blood pressure lowering efficacy, and …

The effect of Mn on mineral stability in metapelites revisited: new a -x relations for manganese-bearing minerals

2014

The a-x relations recently presented in White et al. (, Journal of Metamorphic Geology, 32, 261-286) are extended to include MnO. This provides a set of internally consistent a-x relations for metapelitic rocks in the MnO-Na2O-CaO-K2O-FeO-MgO-Al2O3-SiO2-H2O-TiO2-O2 (MnNCKFMASHTO) system. The mixing parameters for the Mn-bearing minerals were estimated using the micro-ϕ{symbol} approach of Powell et al. (, Journal of Metamorphic Geology, 32, 245-260). Then the Mn-end-member thermodynamic properties were calibrated using a database of co-existing minerals involving literature data from rocks and from experiments on natural materials. Mn-end-members were calibrated for orthopyroxene, cordierit…

Eine Subfaziesfolge der Grünschieferfazies in den Mittleren Cévennen (Dép. Ardèche) mit Pyrophyllit aufweisenden Mineralparagenesen

1969

In the middle part of the Cevennes, situated at the south-eastern border of the Massif Central, there are outcrops of micaschists, gneisses and granites in which the increase of metamorphic grade was investigated along the river Beaume. This paper presents the results of the petrographic-geochemical analysis of the micaschists of the greenschist facies. Chemical analyses of the micaschist-samples examined petrographically show their nearly constant composition, a fact which e.g., is expressed by similar CaO/Na2O- and MgO/FeO-proportions. As the average values of the determined oxides approach the respective average values of analyses of graywackes recorded by Pettijohn (1957) the original s…

Modernità, secolarizzazione e metafisica. In dialogo (postumo) con Sergio Sarti

2022

Tubular markers do not predict the decline in glomerular filtration rate in type 1 diabetic patients with overt nephropathy.

2011

Recent studies have shown that both glomerular and tubulointerstitial damage are important factors in the pathophysiology and progression of diabetic nephropathy. To examine whether markers of tubular damage are useful in monitoring the progression of disease, we measured urinary levels of neutrophil gelatinase-associated lipocalin (NGAL), liver-fatty acid-binding protein (LFABP), and kidney injury molecule-1 (KIM-1) in a 3-year intervention study of 63 type 1 diabetic patients with kidney disease. The baseline mean glomerular filtration rate (GFR) was 87 ml/min per 1.73 m(2) and urinary albumin excretion 1141 mg/24 h. Patients with the highest compared with the lowest quartile of urinary N…

Are radiological endpoints surrogate outcomes of overall survival in hepatocellular carcinoma treated with transarterial chemoembolization?

2021

Background& Aims: Time to progression (TTP) and progression-free survival (PFS) are commonly used as surrogate endpoints in oncology trials. We aimed to assess the surrogacy relationship of TTP and PFS with overall survival (OS) in studies of transarterial chemoembolization (TACE) for unresectable hepatocellular carcinoma (u-HCC) by innovative methods. Methods: A search of databases for studies of TACE for u-HCC reporting both OS and TTP or PFS was performed. Individual patient data were extracted from TTP/PFS and OS Kaplan-Meier curves of TACE arms. Pooled median TTP and OS were obtained from random-effect model. The surrogate relationships of hazard ratios (HRs) and median TTP for OS …
