Use of ICS in COPD: From Blockbuster Medicine to Precision Medicine

Chronic obstructive pulmonary disease (COPD) is a major cause of mortality worldwide, whose burden is expected to increase in the next decades, because of numerous risk factors, including the aging of the population. COPD is both preventable and treatable by an effective management including risk factor reduction, prevention, assessment, and treatment of acute exacerbations and co-morbidities. The available agents approved for COPD treatment are long-acting or ultra-long-acting β2-agonists (LABAs) and long-acting muscarinic antagonists (LAMAs) bronchodilators, as well as inhaled corticosteroids (ICS) in combination with LABAs. L’uso delle discariche in Italia supera il 40% dei rifiuti prodotti, due punti in più rispetto alla media europea. A pesare sul dato italiano sono i pessimi dati del sud: Molise e Basilicata 80% circa; Calabria e Puglia 70% circa; Campania 50% circa. Il primato è della Sicilia, che “butta” in discarica il 90% delle materie prime contenute nei rifiuti prodotti.

Acetylcholine (ACh), synthesized by choline acetyltransferase (ChAT), and muscarinic M1, M2, and M3 receptors (MRs) are involved in fibroblast proliferation. We evaluated ChAT, MRs, and extracellular signal-regulated kinase (ERK) 1/2 and nuclear factor (NF) κB activation in lung fibroblasts from patients with chronic obstructive pulmonary disease (COPD), control smokers, and controls. Human fetal lung fibroblasts (HFL-1) stimulated with interleukin (IL)-1β, tumor necrosis factor (TNF)-α, and cigarette smoke extracts (CSEs) were evaluated for ChAT and MR expression. Research on young offenders has primarily focused on identifying predictors of the maladaptive, aggressive behavior; there is a scarcity of evidence on factors that relate to prosocial behavior in these adolescents.
