Definition of tachycardia for risk stratification of pulmonary embolism

Tachycardia is a reliable predictor of adverse outcomes in normotensive patients with acute pulmonary embolism (PE). However, different prognostic relevant heart rate thresholds have been proposed. The aim of the study was to investigate the prognostic performance of different thresholds used for defining tachycardia in normotensive PE patients.We performed a post-hoc analysis of normotensive patients with confirmed PE consecutively included in a single-centre and a multi-centre registry. Sonographic evidence of intraperitoneal fluid. An experimental study and its clinical implications.

In order to evaluate the sensitivity of ultrasound to intraperitoneal fluid, such as ascites or blood, an experimental study was performed in the pig. Various amounts of fluid were injected into the peritoneal cavity to investigate distribution and diagnostic criteria in different positions. As little as 10 ml of fluid was visualized around the urinary bladder in an upright position. In the supine position, 20 ml could be detected around the bladder and 30 ml around the liver. La cultura greca esprime un'attenzione enfatica al problema dell'alterità. Se la differenza di genere costituisce una frontiera invalicabile, rendendo il femminile (insieme con il barbaro) terreno preferenziale di rappresentazione dell'"altro", tanto più questa alterità subisce un'accelerazione quando al femminile si associa il "mostruoso". Non è infatti un caso se nella cultura della Grecia antica il folklore e i racconti per l'infanzia prediligono i mostri femminili, dediti al cannibalismo e soprattutto all'uccisione di bambini e di adolescenti.

Background. Ascites is one of the most frequent severe complications in patients with liver cirrhosis. The treatment of this chronic disease usually requires the prolonged use of albumin, frequently continued even after patients' discharge from the hospital. Aims. Aim of the study was to define a consensus among Italian physicians with regard to the use of albumin in patients with decompensated cirrhosis and ascites. Methods. Intraosseous (i.o.) infusion is considered a useful technique for the administration of medications and fluids in emergency situations when peripheral intravascular access is not possible. This study investigated the effectiveness of i.o. versus intravenous (i.v.) infusion of hydroxyethyl starch (HES 200/0.5) in hypovolemic pigs. Twenty-three pigs (8- to 9-week-old) were anaesthesized, instrumented and blood was withdrawn (25-30 ml/kg) to50 mmHg mean arterial pressure (MAP).
