Automatic scanning of large tissue areas in neurosurgery using optical coherence tomography

Background With its high spatial and temporal resolution, optical coherence tomography (OCT) is an ideal modality for intra-operative imaging. One possible application is to detect tumour invaded tissue in neurosurgery, e.g. during complete resection of glioblastoma. Ideally, the whole resection cavity is scanned. However, OCT is limited to a small field of view (FOV) and scanning perpendicular to the tissue surface. Purpose It is generally accepted that using a video laryngoscope is associated with an improved visualization of the glottis. However, correctly placing the endotracheal tube might be challenging. Endoscope-assisted Craniotomy

We describe a surgical technique to improve visualization of deep brain structures during craniotomy. The technique, developed in a cadaveric model, combines the use of an operating microscope and a solid-rod lens endoscope. Addition of the endoscope during craniotomy allows the operator to visualize structures that otherwise might not have been seen. An approach to the brain stem is demonstrated.
