The anatomy of the heart in the sonogram. A comparison between anatomic and ultrasonic cross-section.

1977

Cardiac sonography has only recently been introduced as a diagnostic procedure. In order to facilitate the interpretation and evaluation of the ultrasonic cardiac cross-section displayed by this two-dimensional technique, we performed anatomic cross-sections corresponding to ultrasonic cross-sections. The ultrasonic cross-sectional images were taken from children with a real-time-motion scanner. The anatomic cross-sections were taken from adult hearts. Two ultrasonic transverse cross-sections are compared with the two corresponding anatomic cross-sections and three ultrasonic longitudinal cross-sections with one corresponding anatomic cross-section. The direct comparison between anatomic an…

Endoscope-assisted keyhole surgery for aneurysms of the anterior circulation and the basilar apex

2000

In the treatment of cerebral aneurysms, preoperative surgical planning and intraoperative strategies for microsurgical approaches are important issues. Within the context of minimally invasive neurosurgery, so-called keyhole approaches, especially those with intraoperative endoscopic assistance, have gained increasing interest, but their application for cerebral aneurysm surgery is a matter of ongoing controversy. The main aspects of keyhole microsurgery, with and without endoscopic assistance, for cerebral aneurysms are explained. To present the keyhole concept, which is based on the preoperative analysis and intraoperative use of pre-existing anatomic windows, this article concentrates on…

Chimioradiothérapie des cancers de l’œsophage : quelles aires ganglionnaires faut-il irradier ?

2014

Esophageal cancer has a high likelihood of distant lymphatic spread even at an early stage. Radiotherapy plays a major role in the management of localized or locally-advanced esophageal cancer with a regional or distant lymph node involvement. Radiotherapy can sterilize micrometastatic nodes and cancer cells in transit in the peri-esophageal fat that are not removed by surgery. After preoperative chemoradiotherapy followed by monobloc esophagectomy including lymph node dissection above and below the diaphragm, the locoregional failure rate was around 3% in the Chemoradiotherapy for Esophageal Cancer followed by Surgery Study Group (CROSS) trial. This is significantly lower than that observe…

Art und Prognose des lokoregionären Rezidivs beim Rektumkarzinom: Eine Nachsorgestudie

2008

The recurrence rate after 438 curative operations for rectal carcinoma was 44.1% (193 cases). It was independent of age and sex, but correlated closely with tumour stage. Tumours in an early stage recurred rarely and late. The success of a second operation was influenced by the type of local-regional recurrence. Only three of 34 presacral recurrences were again treated surgically with cure as the aim, compared with seven of 17 anastomosis recurrences, five of eight perineal recurrences, four of six recurrences after local dissection, and all colostomy recurrences, vaginal recurrences and metachronic second tumours. The further course demonstrated that about three quarters of patients who ha…

2021

Abstract Background Clinical management ranges from surveillance or curettage to wide resection for atypical to higher-grade cartilaginous tumours, respectively. Our aim was to investigate the performance of computed tomography (CT) radiomics-based machine learning for classification of atypical cartilaginous tumours and higher-grade chondrosarcomas of long bones. Methods One-hundred-twenty patients with histology-proven lesions were retrospectively included. The training cohort consisted of 84 CT scans from centre 1 (n=55 G1 or atypical cartilaginous tumours; n=29 G2-G4 chondrosarcomas). The external test cohort consisted of the CT component of 36 positron emission tomography-CT scans from…

Laparoscopic Treatment of Intussusception

2021

Most cases of ileocolic intussusception are reduced by imaging-guided air or fluid enema. If unsuccessful, operative intervention is necessary. The laparoscopic approach allows inspection of the bowel, identification of intraabdominal pathology, and achieves reduction of the intussuscepted bowel in most cases. The procedure requires the combination of slow and gradual traction as well as “rolling” the cecum distally. If unsuccessful, the bowel can be delivered through an extended umbilical incision for extracorporeal manipulation, or resection if this should be necessary.

Die Tonsillotomie mit der bipolaren Koagulationsschere

2008

BACKGROUND: Nowadays, tonsillotomy is an accepted operating procedure for the diagnosis of tonsillar hyperplasia in small children. Up to now many different techniques such as laser, radiofrequency, coblation or micro-debrider have been used. In this study, for the first time bipolar coagulation scissors were utilized. METHODS: Up to December 2006, a total of 91 children with benign tonsillar hyperplasia were tonsillotomized by bipolar coagulation scissors. Of these 91 children, 38 (average age 74 months) were recruited for this study. Pre-operative and post-operative complaints were documented and the success of the operation was evaluated by the parents. RESULTS: In more than 90 % of the …

Kryotherapie primärer und sekundärer Lebertumoren

2002

AIMS We assessed the morbidity, mortality and efficacy of cryotherapy for primary and secondary malignant liver tumours in a prospective case control study. METHODS Since 1996 we performed 77 cryosurgical procedures on 71 patients. 6 patients had hepatocellular carcinoma, the remainders metastases, mainly of colorectal origin (n = 49). Cryotherapy was used when a complete tumour resection was not feasible, but complete tumour destruction by cryotherapy seemed possible. Mean follow-up was 21 months. RESULTS The mean number of lesions per patient was 2.6 (1-10) with a mean maximum diameter of 3.7 cm (1-11). In 36 of 71 patients cryotherapy was used in combination with liver resection. Morbidi…

Expanded forehead (Fricke) flap for large defect of lateral canthal area

1993

The use of an expanded forehead (Fricke) flap to repair large full thickness defects of the lateral canthus and/or of the upper eyelid is presented. The reconstruction is performed in two stages. In the first stage, a rectangular, 25 ml tissue expander (6 cm in width and 2 cm in length) is inserted under the skin lateral to and above the eyebrow; expansion is completed in about three weeks. At the second stage, the lesion is excised, and the laterally based expanded flap is elevated. Using the expanded Fricke flap, thinner, abundant skin is obtained, and the donor site is directly closed, without grafting and without distorting the eyebrow. If there is a shortage of conjunctiva, this can be…
